A medical and work history, despite common perceptions, is probably the most important part of an examination. Most diagnoses of disease in medicine are made by the history. Laboratory tests are nsed to confirm past illnesses and injuries. Doctors are interested in the history of lung, heart, kidney, liver, and other chronic diseases for the individual and family. Doctors will also be concerned abont symptoms indicating heart or lung disease and smoking habits.

In recent years, the three-level system of NDT experts certification, which corresponds to the EN 473, is being introduced m Ukraine The unified Rules of NDT Experts Certification will be introduced in the near future. This work is headed by the State Committee of Ukraine on Labour Safety), and with the aim of the most expedient transition to EN 473, the National Certification Committee of Ukraine on NDT was established by the initiative of US NDT TD. The Committee has the tasks of preparing the programs, procedures, questionnaires for carrying out the certification. It is a non-profit organisation which is in charge of the methodological issues of certification m the US NDT TD. 

The technology package for the toll should be at least partially established prior to starting the active search for a toller. This will assist you in defining the level and areas of expertise needed for the project. Some companies that frequently let toll contracts have internal service groups that assist in the toller selection process. An example of an internal questionnaire used at such a company is shown in Example 2-1. It is an excellent way to identify the expertise required for a specific tolling project. 

Companies desiring to initiate a toll without the luxury of a special services group can adopt the questionnaire for their project development group to use. Candidate tollers may use this example as a preparatory tool for completing bid responses. 

Companies experienced in assessing candidate tollers typically have some type of assessment questionnaire used to collect data. The assessment questionnaire is useful because it provides a relatively inexpensive tool to collect a standard set of data.

The company then uses the specific toller s completed questionnaire again during the site visit to verify that accurate information was provided and resolve open issues. 

Some companies have more detailed forms for the site visit. One example of an assessment questionnaire is provided in Appendix A, Sample Toller Pre-Assessment Qjiestionnaire. 

Companies evaluating tollers should modify the examples in this book to best meet their needs. Tollers may gain insight by comparing the examples to questionnaires that they have completed previously. Additional samples of assessment forms are provided in the appendices. 

Assessment questionnaires may not typically require details about technical expertise the candidate tollers may possess. It is assumed that the list of candidates who will receive the questionnaire have... 

Some of the candidate tollers practices, records and documents can not be assessed using only a questionnaire form. A site visit to one or more of the short-listed candidate tollers is usually indicated. This is especially helpful if an unfamiliar toller is competing against a toller that is currently being used. Candidate tollers can use the information in this section to prepare for a site interview when one is requested. 

The extent to which documents are reviewed will depend upon the results of the initial questionnaire responses, the interviews, and facility walk-around. In general, documents should be reviewed to verify good practice compliance, and a sampling will usually suffice. 

The technical review team should compare the overall response related to process safety, environmental, health, and quality capability-related items to the data gathered through questionnaires or visits. Does their practice match their policy ...

The numbers of dissatisfied persons in Table 6.3 are not additive. Some of the people experiencing general thermal comfort (PMV-PPD) may be the same as the people experiencing local thermal discomfort. In practice, a higher or lower number of dissatisfied persons may be found using subjective questionnaires in field investigations (ISO 10551). 

In 1991, the Verband der Automobilindustrie e. V. (VDA) published VDA 6.1, Quality Spstem Audit, a questionnaire on quality system evaluation based on DIN EN ISQ 9004. VDA 6 is a series of guides covering the basics for quality audits, auditing, and certification. They were therefore not intended as supplementary requirements to ISQ 9000 but as guides for auditors performing audits of automotive suppliers. Their intention was to improve auditor competency in the industry by providing a uniform interpretation of ISQ 9000 requirements and a common approach to automotive audits. VDA 6.1 has been revised several times and is currently in its fourth edition. There are nine volumes in the series (see Appendix A). 

This questionnaire addresses all the key requirements of ISO/TS 16949 and will help you determine the margin between where you are now and where you need to be to achieve ISO/TS 16949 registration. Next is a questionnaire which only covers the specific requirements of the standard. This breaks down the requirements into their individual components where it is likely that the solutions for each part will be different. It can be used as a basic checklist for verifying that the quality system you have designed addresses all the requirements, or as a means of creating policy or of assessing conformity.
