Questionnaires submission

The Board transmits form D, an annual questionnaire on substances frequently used in the illicit manufacture of narcotic drugs and psychotropic substances, to all Governments. As at 1 November 2004, a total of 135 States and territories, as well as the European Commission on behalf of the States members of the European Union, had submitted form D for 2003 (see annex II for details). With 66 per cent of all States parties and 48 per cent of non-parties submitting data for 2003, the rate of submission for 2003 is approximately the same as that of previous years. 

Prior to a site visit, assess the vendor remotely through product literature and submission of a preaudit questionnaire. When a vendor site audit becomes necessary, plan the audit s scope and focus and identify the audit team. Audit team members should include user group lead, information services groups (IT/IS/MIS), compliance, purchasing, and validation personnel. 

Application Form. In most Gulf States except a few (e.g., Bahrain and the UAE where the Health Authority has provided a check list), the local Health Authority has an appropriate application form (questionnaire), which has to be duly completed by the official local agent and forwarded along with the submission. In Kuwait (Kuwait MOH 1990) and Qatar the application is uncomplicated, more or less a questionnaire with approximately 20 questions. On the contrary, in Oman the application form is more elaborate. 

For qualitative measurements, we used Likert-scale questionnaires about the users perceived difficulty level of specific actions with an interface, such as note-taking, submission, and erasing. For quantitative measurements, we instrumented the two interfaces, and logged important events such as pen property changes, strokes, gestures, slide navigation, and submission.
