In recent years, the three-level system of NDT experts certification, which corresponds to the EN 473, is being introduced m Ukraine The unified Rules of NDT Experts Certification will be introduced in the near future. This work is headed by the State Committee of Ukraine on Labour Safety), and with the aim of the most expedient transition to EN 473, the National Certification Committee of Ukraine on NDT was established by the initiative of US NDT TD. The Committee has the tasks of preparing the programs, procedures, questionnaires for carrying out the certification. It is a non-profit organisation which is in charge of the methodological issues of certification m the US NDT TD. 

In 1991, the Verband der Automobilindustrie e. V. (VDA) published VDA 6.1, Quality Spstem Audit, a questionnaire on quality system evaluation based on DIN EN ISQ 9004. VDA 6 is a series of guides covering the basics for quality audits, auditing, and certification. They were therefore not intended as supplementary requirements to ISQ 9000 but as guides for auditors performing audits of automotive suppliers. Their intention was to improve auditor competency in the industry by providing a uniform interpretation of ISQ 9000 requirements and a common approach to automotive audits. VDA 6.1 has been revised several times and is currently in its fourth edition. There are nine volumes in the series (see Appendix A). 

Next is a questionnaire which only covers the specific requirements of the standard. This breaks down the requirements into their individual components where it is likely that the solutions for each part will be different. It can be used as a basic checklist for verifying that the quality system you have designed addresses all the requirements, or as a means of creating policy or of assessing conformity. 

Surveys often involve questionnaires that are completed by facility staff. Surveys can be used in place of any on-site audits however, this precludes your gaining first-hand Imowledge of the PSM systems and leaves you unable to vouch for the accuracy of the information gathered. 

Questionnaires can be used by a facility to conduct a self-assessment or can be administered by an outside evaluator, by phone or in person. In any case the questionnaire typically focuses on identifying PSM activities and program content based on the requirements of the PSM model selected, and assessing condensed criteria for the effectiveness of management systems (as shown in Figure 4-2, page 78). 

IWo basic types of questionnaires can be used yes/no or open-ended. A sample yes/no questionnaire is provided in Figure 4-6 Figure 4-7 displays an open-ended version. The yes/no questionnaire confirms the existence of specific PSM program components or management system characteristics, while an open-ended questionnaire requires an explanation of the response and can be used to invite discussion. Yes/no questionnaires can be filled out by the facility or can be administered by less experienced assessors. Open-ended questionnaires require more experienced assessors, who are equipped to guide discussion and probe for further information. 

To make the most of our participation in the PSM pilot test, the PSM Task Force needs your help. One of the main reasons we are conducting this test is to make sure our new system for managing process safety is practical and that it gives each of us what we need to do our jobs safely and effectively. We can t do this without your input. That s why the pilot test plan calls for getting your feedback on a continuing basis, and one of the ways we ll be asking for it is through questionnaires like this one. 

Man/vood employees, on the whole, continue to be helpful and cooperative, despite some initial pockets of resistance. Employee feedback confirms that the confusion reported at the outset had more to do with our communications skills than the substance of the PSM system. Even though 1rain the trainer had some early glitches, this concept has proven very popular with Marwood personnel a number of questionnaires single this out as a program they would like to see replicated in other areas. 

Hybrid systems are those systems that employ various strategies to collect data. In such systems, data may be collected on paper forms as patient self-administered questionnaires, while additional data may be downloaded from centralized databases. 

Partti-Pellinen et al. (1996) used a cross-sectional, self-administered questionnaire to assess the eye, respiratory tract, and central nervous system symptoms experienced by adults in a slightly polluted and a reference community. In the polluted community the mean annual TRS concentrations were 2-3 g/m3, the 24-hour average concentrations varied between 0-56 g/m3, and the maximum 1-hour concentration was 155 g/m3 there was no TRS detected in the reference community. In the polluted community, the sulfur dioxide annual mean concentration was 1 g/m3, the 24-hour average concentrations varied... 

Gathering audit data can be accomplished through observations, documents, and interviews. The data obtained is used to verify and validate that the process safety management systems are implemented and functioning as designed. Data gathering can be aided oy the use of audit samples, where a representative number of items are audited to draw a conclusion, and by using self-evaluation questionnaires.

In this study, environmental and resource use impacts of organic farming are assessed relative to conventional farming systems. The primary source of information for this report is a survey of specialists in 18 European countries (all EU-member states plus Norway, Switzerland and the Czech Republic) using a structured questionnaire. These experts were asked to refer back to their national literature on the subject. The second important source of information used in this report is a literature search in international databases completed by the authors. 

The following is a summary of once-through cooling system flow rates based on responses to 308 USEPA questionnaires. 

As in previous years, the Report is based on data obtained primarily from the Annual Reports Questionnaire (ARQ) sent by Governments to UNODC in 2006, supplemented by other sources when necessary and where available. Two of the main limitations herein are (i) that ARQ reporting is not systematic enough, both in terms of number of countries responding and of content, and (ii) that most countries lack the adequate monitoring systems required to produce reliable, comprehensive and internationally comparable data. National monitoring systems are, however, improving and UNODC has contributed to this process.
