In recent years, the three-level system of NDT experts certification, which corresponds to the EN 473, is being introduced m Ukraine The unified Rules of NDT Experts Certification will be introduced in the near future. This work is headed by the State Committee of Ukraine on Labour Safety), and with the aim of the most expedient transition to EN 473, the National Certification Committee of Ukraine on NDT was established by the initiative of US NDT TD. The Committee has the tasks of preparing the programs, procedures, questionnaires for carrying out the certification. In 1991, the Verband der Automobilindustrie e. V. (VDA) published VDA 6.1, Quality Spstem Audit, a questionnaire on quality system evaluation based on DIN EN ISQ 9004. VDA 6 is a series of guides covering the basics for quality audits, auditing, and certification. They were therefore not intended as supplementary requirements to ISQ 9000 but as guides for auditors performing audits of automotive suppliers. Their intention was to improve auditor competency in the industry by providing a uniform interpretation of ISQ 9000 requirements and a common approach to automotive audits. VDA 6.1 has been revised several times and is currently in its fourth edition. There are nine volumes in the series (see Appendix A). [Pg.6]

Next is a questionnaire which only covers the specific requirements of the standard. This breaks down the requirements into their individual components where it is likely that the solutions for each part will be different. It can be used as a basic checklist for verifying that the quality system you have designed addresses all the requirements, or as a means of creating policy or of assessing conformity. [Pg.86]

IWo basic types of questionnaires can be used yes/no or open-ended. A sample yes/no questionnaire is provided in Figure 4-6 Figure 4-7 displays an open-ended version. The yes/no questionnaire confirms the existence of specific PSM program components or management system characteristics, while an open-ended questionnaire requires an explanation of the response and can be used to invite discussion. Yes/no questionnaires can be filled out by the facility or can be administered by less experienced assessors. Gathering audit data can be accomplished through observations, documents, and interviews.
