Safety perception questionnaire

One means of gathering data for monitor indicators is patient safety culture questionnaires. We have developed a patient safety culture questionnaire, TUKU, that measures employees perceptions of the organisational functions depicted in Table 9.4 as well as employees psychological states, such as sense of control and worry about patient safety (Reiman et al. 2013). In one hospital, the results of the questionnaire were compared with the ratio of patient safety incidents at the hospital s 40 units 16 months after the safety culture questionnaire was administered. The results, which must be treated with caution due to the small sample size, indicated that perceptions of work process management , work conditions management . 

Likert invented a way to measure climate with a forced choice questionnaire that he administered to employees to find out their perception of how good the company is in the ten areas. He later took the perception survey results and ran correlation studies with things like profitability, return on investment, growth, and other bottom-line figures, invariably coming up with extremely high positive correlation. Apparently, climate determines results [3,4]. We will discuss employee safety perception and how it will affect the operation in more detail in Chapter 17.

The general consensus within the literatnre is that assessing employee attitudes alone is not satisfactory when wishing to measnre an organization s safety culture, as they can only elicit safety perceptions and attitndes (HSE, 2005a). Other methods that can be used in addition to safety culture questionnaires include observational studies, focus groups, interviews, case studies, surveys, and qnestionnaires. 

Assess. Survey the culture to determine prevailing attitudes, beliefs, and behaviors related to safety and the environment by way of confidential questionnaires (perception survey), interviews (independent review process), focus group meetings, and visits to assess workplace conditions and possible contributions to incidents [4]. We will cover this in more detail in Chapter 17. 

Perception surveys have also been used to provide measurements of the quality of safety management systems in place. Through an interview system or the completion of surveys, they provide management with a picture of what employees at various levels of responsibility think about the management of safety in the organization. Perception surveys are outcome surveys in that the perceptions people have of how safety is managed derive from their observations of what got done or didn t get done with respect to the safety system elements covered in the interview or questionnaire process. 

Refer to Appendix U, Safety Management Percephon Questionnaire for an example of a perception survey, an instrument provided by Bob Lapidus and Dr Mike Waite that provides a validated format that has been used to gain insights from employees (Lapidus Waite, 2013). 

Purpose This questionnaire is designed to identify your perceptions about your organization s current occupational safety and health program. 

The paper presents and discusses the results of this survey. Results illustrate that all organizations have been using safety management policies and their safety performance metrics are positive, however all have limitations. The non-proof of Hypothesis 2 (the relationship between the number of accidents and safety training) shows that the small size of the establishments has an impact on the perceptions of the employees. It also serves as an example that, in the future, a more expanded version of the questionnaire should be used in order to avoid similar shortcomings.

The elements that should be considered in evaluating the safety level were introduced in the previous section. The elements must be quantified to assess the risk associated with a navigation area based on navigators perceptions. Hence, a questionnaire was used to obtain input from navigators. The design of the questionnaire used to explore navigators perceptions is shown in table 2. 

In the questionnaire, navigators were asked how much each element affects their perception, using a nine-level evaluation scale (level 1 no influence level 9 significant influence). The results reflect the navigators opinions in a quantitative manner that can be incorporated into the safety evaluation model.
