Questionnaire checklist is a form of scahng checkhst but uses a series of carefully directed questions to elicit information about possible impacts and their hkely importance. 

In this study, the researchers focus on SMEs in the manufacturing sector, the sampled group size was 100 SMEs, and data were collected by quota sampling. With the above sampling groups, a questionnaire was used as a tool for data collection to create a questionnaire checklist by smdying twenty-four variables under all the five performance measurement perspectives (quality, time, finance, and customer satisfaction) for performance measurement of SMEs in the manufacturing sector in the province. 

A simple health monitoring system should be agreed with employees or their representatives that includes a questionnaire checklist (available on the HSE website) to be completed once a year by employees at risk.

Additional examples of variability in data collection (which, in turn, affects data interpretation) include questionnaires and physical exam forms. Each subject who showed clear-cut improvement on the MSLT (usually to an optimal score of 20) had obtained substantial extra sleep. Recording audit observations is an essential part of the audit to enable the auditor to prepare detailed, accurate and complete audit reports which are based on factual observations.
