Companies evaluating tollers should modify the examples in this book to best meet their needs. Tollers may gain insight by comparing the examples to questionnaires that they have completed previously. Additional samples of assessment forms are provided in the appendices. 

In 1991, the Verband der Automobilindustrie e. V. (VDA) published VDA 6.1, Quality Spstem Audit, a questionnaire on quality system evaluation based on DIN EN ISQ 9004. VDA 6 is a series of guides covering the basics for quality audits, auditing, and certification. They were therefore not intended as supplementary requirements to ISQ 9000 but as guides for auditors performing audits of automotive suppliers. Their intention was to improve auditor competency in the industry by providing a uniform interpretation of ISQ 9000 requirements and a common approach to automotive audits. VDA 6.1 has been revised several times and is currently in its fourth edition. There are nine volumes in the series (see Appendix A). 

Questionnaires can be used by a facility to conduct a self-assessment or can be administered by an outside evaluator, by phone or in person. In any case the questionnaire typically focuses on identifying PSM activities and program content based on the requirements of the PSM model selected, and assessing condensed criteria for the effectiveness of management systems (as shown in Figure 4-2, page 78).

Self-report questionnaires containing a mixture of open and forced-choice questions. These could be distributed amongst mentors, mentees, relationship supervisors, line managers and program co-ordinators. In addition, you could also use this method to consult with peers, customers and/or suppliers -i.e. anyone who could provide you with valuable evaluation data. Make sure, however, that you customize the questions to the group of respondents you are addressing.

In most studies, phytoestrogen intake has been estimated by direct methods that evaluate food intake either by recall (food-frequency questionnaires -FFQs) or by record (food diary), and subsequently by composition databases based on information of this kind. Gathering audit data can be accomplished through observations, documents, and interviews. The data obtained is used to verify and validate that the process safety management systems are implemented and functioning as designed. Data gathering can be aided oy the use of audit samples, where a representative number of items are audited to draw a conclusion, and by using self-evaluation questionnaires.
