As with any power equipment, gas turbines require a program of planned inspections with repair or replacement of damaged components. A properly designed and conducted inspection and preventive maintenance program can do much to increase the availability of gas turbines and reduce unscheduled maintenance. Inspections and preventive maintenance can be expensive, but not as costly as forced shutdowns. Nearly all manufacturers emphasize and describe preventive maintenance procedures to ensure the reliability of their machinery, and any maintenance program should be based on manufacturer s recommendations. Inspection and preventive maintenance procedures can be tailored to individual equipment application with references such as the manufacturer s instruction book, the operator s manual, and the preventive maintenance checklist. 

At Site I, the prime contractor s SSAHP required daily site inspections, the documentation of safety and health deficiencies, and the abatement of deficiencies. Records of site deficiencies, however, were kept intermittently, and hazard abatement was not documented. The subcontractor s SSAHP did not address site inspections and hazard abatement, but its TSCA permit application included requirements for site inspections. Inspection documentation, however, was not available on site, and the site manager was unaware of these written requirements. The site manager did, however, indicate that he conducted site inspections using a mental checklist and that he conducted inspections of remediation equipment before each use. 

The audit includes a review of the process safety information, inspection of the physical facilities, and interviews with all levels of plant personnel. Using the procedures and checklist, the team systematically analyzes compliance with the PSM Rule and any other relevant corporate policies. The training program is reviewed for adequacy of content, frequency and effectiveness of training. Interviews determine employee knowledge and awareness ofthe safety procedures, duties, rules, and emergency response assignments. Inspections consist of a visual check of the fire protection systems by qualified personnel to ensure the system is available for immediate use. Visual checks should be made using a checklist. The inspection should verify that the fixed and portable equipment is ready to be operated. The associated devices, fittings, piping, and valving are inspected to ensure they have not been tampered with and that there is no obvious deterioration, physical damage, or condition to prevent operation. The date the inspection was performed and the initials of the person performing the inspection should be recorded. Records should be kept on a tag or label attached to the fire extinguisher, on an inspection checklist maintained on file, or in an electronic system that provides a permanent record. 

Checklists are a valuable tool for documenting the inspection, test, and maintenance of fire protection equipment and systems. The CD-ROM accompanying this book contains checklists developed by one company. Many small and medium size companies may not have the resources to develop these checklists. 

Experienced senior staff of the sponsor must always visit the investigator site before a new clinical trial starts, even if the investigator has been involved in previous studies. Most pharmaceutical companies have checklists and SOPs of the requirements of an investigator site. Key questions will need to be answered relating to staff support and the present workload of the site. The competence of the staff to conduct any procedures, the maintenance, calibration and QC of any equipment to be used, and whether other clinical trials demand too much resource are aU questions that need answers. Periodic review of the calibration system should happen once a year using an established inspection checklist. 

QAI will monitor the results of pressure differentials, temperature, and humidity obtained from the control room daily before starting the manufacturing process in the area for comparison of results with the standard provided for each area. They should be within limits. Whenever any operation such as manufacturing or filling starts, QAI will fill an inspection start-up checklist and give release for manufacturing or packaging. The checklist is divided into three areas 1) Material Storage and Handling, 2) Production Processes and 3) Treatment Alternatives. Material degradation, samples, spills, storage, and inspections are addressed in the first section. Production processes include source reduction for process bath solutions and rinse systems, resource recovery and recycling, and solvent management. Treatment Alternatives focuses on process water pretreatment, modification of conventional wastewater treatment, and alternative waste treatment methods. Each of these subsections follow the question and answer format. 

Servicing standards will also provide advice on safety, checklists for inspections, product operating conditions, inhibitor reserves to be maintained, and provide methods of sampling, analysis, monitoring, reporting, etc. 

The procedure would typically include an inspection checklist, calibration procedure, test equipment stipulations, and documentation requirements (e.g., inspection certificates, calibration certificates, hazardous area certification, EMC/RFI certification, material certificates).
