Self-Inspection Checklists

The PIC/S harmonized computer inspection gnide contains aide memoires that can be nsed as inpnt to develop a self-inspection checklist. Snch checklists can be used to examine any aspect of working practices, assess the level of GxP compliance of processes, including compnter systems, with a view to identifying poor practices and opportnnities for improvement. They can also be nsed with eqnal beneficial effect in non-GxP areas. 

Phase 2. Follow-up Inspection. After reviewing the self-inspection checklists for a specific group or facility, CCHASP schedules a follow-up inspection of the area. Problems identified in the self-inspection process (phase 1) establish the basis for an in-depth inspection of the laboratory or facility. The CCHASP inspector uses the same checklists used in the self-inspection phase to verify compliance with environment, health and safety requirements and to point out problems that may have been overlooked. Rather than trying to be comprehensive for each area, it is probably better to focus on one or two areas and just hit the highlights of the others. Change the focus points on the next inspections. Remember that your purpose is to make your lab safer by finding and fixing previously unrecognized hazards so that they are managed. Also, remember that the checklist is for you and your lab mates, but as with all lab notes it is prudent to keep this as a reminder of when and what you did on your last few self-inspections. 

During an internal audit or self-inspection, trained employees of the organisation check the functioning of the quality system. They may use structured questions and checklists with which they compare the information in the quality manual and connected procedures with the actual situation. An experienced auditor will also use a more intuitive approach for instance getting information about (lack of) cooperation between sections or departments, exploring peculiarities or interviewing staff. When discrepancies are found, they will usually be classified as critical, important or minor. Subsequently corrective or preventive measures (see Sect. 35.6.15) should be taken to adapt the actual methods or the documentation in order to make them match again.

Ideally, each lab or facility group completes the self-inspection process every year. By allowing researchers to complete their own checklists prior to an actual inspection, we introduce a valuable element of training into the inspection program. 

Site self-inspection reports along with corrective action are necessary. If a checklist is used for site surveys, a narrative section on each form must also be conq)leted. 

With these seven steps, an organization can cooperatively prepare for an FDA investigation, can appropriately assume the self-regulatory responsibility, and can assure the quality control appropriate to the biomedical industries. A checklist for conducting a self-regulatory GLP inspection is also provided. 

The commonest way to introduce order into safety inspection is to use checklists, and there are many examples—most textbooks on safety contain at least one. Although the examples can be used as models, the best checklists are those constructed for a particular workplace. The basis for them all, though, is much the same. They comprise a list of types of hazard, together with a list of how hazards could manifest themselves. Within a museum workshop, the main types of hazard are likely to be fire, chemicals, electricity, radiation, machinery, handling equipment, micro-organisms and the general work layout. Most of these are self-evident from a quick inspection of the workplace.
