In summary, early evidence appears to confirm the assumption that antidepressants, which successfully effect behavioral change in those with bulimia, will also do so in patients with BED. After discontinuation of a brief course of an antidepressant, binging frequency and severity often resume at pretreatment levels. The longterm efficacy of continued autidepressaut administration is unknown.

The therapeutic effects of psychopharmaceuticals are expressed as changes in well-being and behavior. The clinical testing of new substances therefore involves the reliable determination of these modifications in a form that can subsequently undergo statistical analysis. Three basic types of methods are available for this purpose questioning of the patients themselves (or other procedures to determine subjective effects) behavioral observations of various degrees of complexity and test procedures for specific mental functions.

That situation has changed with the development of clomipramine and the SSRIs, which appear to have unique efficacy in treating OCD when compared with other types of psychotropics (see Chapter 13, Obsessive-Compulsive Disorder ). In addition, more effective behavioral approaches have been developed, which, in combination with medication, can substantially ameliorate this condition. 

OCD is known to respond to serotonergic antidepressants. It is characterized by repetitive anxiety-provoking thoughts (obsessions) or repetitive behaviors aimed at reducing anxiety (compulsions). Clomipramine and several of the SSRIs are approved for the treatment of OCD, and they are moderately effective. Behavior therapy is usually combined with the antidepressant for additional benefits.

Despite the extent of inhalant abuse and its potential for destroying mental and physical functions, little is known about effective behavior modification or drug-abuse prevention approaches to treating inhalant abusers. Inhalant abusers are often excluded from more general studies on drug abuse.

Other somatic therapies were developed in the first half of the twentieth century, with variable results. Malaria therapy was conceived in 1917, insulin shock in 1927, psychosutgery in 1936, and electroconvulsive treatment (ECT) in 1938. All of these methods, as originally conceived, carried serious risks, and most demonstrated marginal effectiveness. I Disadvantages. Dose adjustments are needed for patients with decreased renal functioning, and slower dose escalation may be needed to avoid CNS adverse effects. Behavioral problems may hmit therapy in some patients. Currently, there is no parenteral formulation, but one is in development.
