Locatjonfsl - For each hazardous substance that you are reporting, provide a brief narrative description of its onsite location, such as, "first floor stockroom at rear of workshop", "warehouse", "tank farm", "garage", "storage area for flammables, behind garage". What-if produces a table of narrative questions and answers suggesting accident scenano.s. consequences, and mitigation. The results of a What-If/Checklist analysis are documented like the results of a What-lf analysis as a table of accident scenarios, consequences, safety levels, and action items. Design and development plans need to identify the activities to be performed, who will perform them, and when they should commence and be complete. One good technique is to use a network chart (often called a PERT chart), which links all the activities together. Alternatively a bar chart may be adequate. In addition there does need to be some narrative, as charts rarely convey everything required. 

Records can take various forms reports containing narrative, computer data, forms containing data in boxes, graphs, tables, lists, and many others. Where forms are used to collect data, they should carry a form number and name as their identification. When completed they should carry a serial number to give each a separate identity. Records should also be traceable to the product or service they represent and this can be achieved either within the reference number or separately, providing the chance of mistaken identity is eliminated. The standard does not require records to be identifiable to the product involved but unless you do make such provision you will not be able to access the pertinent records or demonstrate conformance to specified requirements. 

A Safety Case is a narrative that literally makes the case that an adequate level of safety has been reached for an installation. It requires looking at all potential hazards which could lead to a loss of the installation, a loss of life, or a major pollution event. A risk analysis is performed on each hazard evaluating the probability of the event occurring and describing the magnitude of the consequences. A discussion is then given of the measure undertaken to lower the probability of occurrence or to mitigate the consequences and a case is made that the risk for the installation meets the ALARP safety criteria. 

A questions-and-answer format, or an Interview with the Chairman (or other senior sponsor)" can help present PSM information and messages in a conversational manner that many readers find more interesting than a more static narrative. 

At this point in the process, you may find it useful to review the preliminary plan you presented for initial management approval (see Chapter2). That "first take" necessarily lacked the detail you are now able to provide. For example, your initial estimates of time requirements might have predicted 18 months for system design and installation now you reali2e that 24 months will be needed. While the initial plan will require changes, it should have components (organizational, stylistic, narrative) that can be adapted for use in the formal plan.

Sometimes it is necessary to review the narrative in raw data records to determine whether a failure has occurred, to establish failure modes and severities, and to see if a record is a duplicate or new failure. Often, the narrative section is the only way the data analyst can determine if the document, especially a work order, is for a legitimate failure, routine maintenance, or a specified test.

Data gathered in the lessons were used to produce case studies for each of the groups of students because such case studies yield rich descriptions of events that are presented in a chronological narrative that incorporates the researcher s observations.
