Topical systems preparation

There are hundreds of topical steroid preparations that are available for the treatment of skin diseases. In addition to their aforementioned antiinflammatory effects, topical steroids also exert their effects by vasoconstriction of the capillaries in the superficial dermis and by reduction of cellular mitosis and cell proliferation especially in the basal cell layer of the skin. In addition to the aforementioned systemic side effects, topical steroids can have adverse local effects. Chronic treatment with topical corticosteroids may increase the risk of bacterial and fungal infections. A combination steroid and antibacterial agent can be used to combat this problem. Additional local side effects that can be caused by extended use of topical steroids are epidermal atrophy, acne, glaucoma and cataracts (thus the weakest concentrations should be used in and around the eyes), pigmentation problems, hypertrichosis, allergic contact dermatitis, perioral dermatitis, and granuloma gluteale infantum (251). [Pg.446]

Topical NSAID preparations are used infrequently in North America. Theoretically, administration via a topical vehicle targets the joints involved and decreases systemic exposure. Randomized, controlled trials, typically of less than 4 weeks duration, have suggested that topical NSAIDs are superior to placebo in relieving OA pain in the first 2 weeks of treatment, but the effect may decline over time.33 Moreover, data are lacking to quantify the theoretical safety advantage of administering NSAIDs topically. [Pg.889]

Rhinocort Aqua and Nasonex are preparations containing topical nasal corticosteroids (budesonide and mometasone furoate respectively). Otrivine contains a nasal decongestant (xylometazoline) and Sudafed is a systemic preparation containing a nasal decongestant (phenylephrine). It has been observed that enantioselective polymer-bound catalysts prepared by copolymerization produce in some cases better asymmetric inductions than systems prepared by grafting [175]. Once an organism has grown on culture, sensitivity testing can be performed to determine which antibiotics are the most effective.The most commonly used method, the Kirby-Bauer diffusion disc system, usually takes 48 hours to perform. Unfortunately, it can be inaccurate because of the lower concentrations of antibiotic on the test discs compared with levels that can be achieved in the cornea through topical application. Tetracaine (pontocaine), a long-acting amino ester, is significantly more potent and has a longer duration of action than procaine. Tetracaine may exhibit increased systemic toxicity because it is more slowly metabolized than the other commonly used ester local anesthetics. It is widely used in spinal anesthesia when a drug of long duration is needed. Tetracaine also is incorporated into several topical anesthetic preparations. Vidarabine is used mainly in human HSV-1 and HSV-2 encephalitis, decreasing the mortality rate from 70 to 30%. Whitley et al. (57) reported that early vidarabine therapy is helpful in controlling complications of localized or disseminated herpes zoster in immunocompromised patients. Vidarabine also is useful in neonatal herpes labialis or genitalis, vaccinia virus, adenovirus, RNA viruses, papovavirus, CMV, and smallpox virus infections. Given the efficacy of vidarabine in certain viral infections, the U.S. FDA approved a 3% ointment for the treatment of herpes simplex keratoconjunctivitis and recurrent epithelial keratitis, and a 2% IV injection for the treatment of herpes simplex encephalitis and herpes zoster infections (Table 43.3). A topical ophthalmic preparation of vidarabine is useful in herpes simplex keratitis but shows little promise in herpes simplex labialis or genitalis. The monophosphate esters of vidarabine are more water-soluble and can be used in smaller volumes and even intramuscularly. These esters are under clinical investigation for the treatment of hepatitis B, systemic and cutaneous herpes simplex, and herpes zoster virus infections in immunocompromised patients. [Pg.1884]

Although topical phenothiazine antihistamine preparations such as promethazine hydrochloride (Phenergan) cream are no longer used in the United States, such topical antihistamine preparations are still widely used in Europe. Individuals who have become sensitized by such topical phenothiazine antihistamines often suffer a flare of the dermatitis when a phenothiazine antihistamine is taken, i.e., a systemic eczematous contact dermatitis. In addition, many individuals acquire allergic sensitization to various phenothiazine drugs which show cross-reactions with the phenothiazine antihistamines. NitrofuraZone. 2-[5-Nitro-2-furanyl)methylene]hydrazinecarboximide, the first nitrofiiran to be employed clinically, is prepared from 5-nitro-2-furancarboxaldehyde and semicarbazide (19). AB cements are not only formulated from relatively small ions with well defined hydration numbers. They may also be prepared from macromolecules which dissolve in water to give multiply charged species known as polyelectrolytes. Cements which fall into this category are the zinc polycarboxylates and the glass-ionomers, the polyelectrolytes being poly(acrylic acid) or acrylic add copolymers. The interaction of such polymers is a complicated topic, and one which is of wide importance to a number of scientific disciplines.
