The type of lesion and whether the patient also has psoriatic arthritis are important issues in cleterinining therapy. Steroid topical creams, cyclosporine andmethotraxate are useful in treatment of psoriasis. Oral tazarotene, a non-biologic retinoid is pending FDA approval for moderate to severe psoriasis. As with most autoimmune disorder s, different patients respond differently and newer more targeted therapies are important goals. [Pg.290]

Eye. Toxic cataract can be due to chloroquine and related drugs, adrenal steroids (topical and systemic), phenothiazines and alkylating agents. Comeal opacities occur with phenothiazines and chloroquine. Retinal injury occurs with thioridazine (particularly, of the antipsychotics), chloroquine and indomethacin. [Pg.146]

Lupus er/thennatosus (affecting tlic skin) Photoprotection is essential. Potent adrenal steroid topically or intralesionally. Hydroxychloroquine or mepacrine. Monitor for retinal toxicity when treatment is long-term.Other agents include auranofin, aceiretin and in severe chilblain LE. thalidomide. A systemic disease, but discoid iupus erythematosus typicaily has no systemic manifestations. [Pg.311]

Uses Bufexamac is a non-steroidal topical medication used for treatment of a variety of problems in which the skin is inflamed. These vary widely from insect bites to burns, plant stings and a wide variety of medical conditions including psoriasis, hemorrhoidal symptoms, eczema and inflammation of the skin following radiotherapy. In this last case, it can be used before radiotherapy to help prevent inflammation of the skin caused by radiation therapy A... [Pg.1199]

There are hundreds of topical steroid preparations that are available for the treatment of skin diseases. In addition to their aforementioned antiinflammatory effects, topical steroids also exert their effects by vasoconstriction of the capillaries in the superficial dermis and by reduction of cellular mitosis and cell proliferation especially in the basal cell layer of the skin. In addition to the aforementioned systemic side effects, topical steroids can have adverse local effects. Chronic treatment with topical corticosteroids may increase the risk of bacterial and fungal infections. A combination steroid and antibacterial agent can be used to combat this problem. Additional local side effects that can be caused by extended use of topical steroids are epidermal atrophy, acne, glaucoma and cataracts (thus the weakest concentrations should be used in and around the eyes), pigmentation problems, hypertrichosis, allergic contact dermatitis, perioral dermatitis, and granuloma gluteale infantum (251). [Pg.446]

Inhaled steroids (commonly used are beclomethasone, budesonide, triamcinolone, fluticasone, flunisolide) appear to attenuate the inflammatory response, to reduce bronchial hyperreactivity, to decrease exacerbations and to improve health status they may also reduce the risk of myocar dial infar ction, but they do not modify the longterm decline in lung function. Whether- steroids affect mortality remains unclear. Many patients appear to be resistant to steroids and large, long-term trials have shown only limited effectiveness of inhaled corticosteroid ther apy. Certainly, the benefit from steroids is smaller in COPD than in asthma. Topical side-effects of inhaled steroids are oropharyngeal candidiasis and hoarse voice. At the normal doses systemic side-effects of inhaled steroids have not been firmly established. The current recommendation is that the addition of inhaled gluco-coiticosteroids to bronchodilator treatment is appropriate for patients with severe to veiy sever e COPD. [Pg.365]

Corticosteroids a chronic painless myopathy associated with the long-term use of corticosteroids is a particularly common example of drug-induced muscle disorder. It is almost certain that mild cases are overlooked because steroids are so frequently used to treat inflammatory myopathies such as polymyositis. Fluorinated steroids are particularly frequently implicated, and the incidence of drug-induced muscle disease is dose and time-related. The presence of muscle weakness can even complicate topical steroid therapy. Corticosteroid-induced myopathy is mediated via intramuscular cytosolic steroid receptors. The steroid-receptor complexes inhibit protein synthesis and interfere with oxidative phosphorylation. The myopathy is associated with vacuolar changes in muscle, and the accumulation of cytoplasmic glycogen and mitochondrial aggregations. [Pg.344]

Hydroquinone is usually more effective when utilized in combination with other agents such as topical retinoids alone or topical retinoids and topical steroids (see Table 14.1). The addition of a weak topical steroid reduces the irritant effect of hydroquinone, but the treat-... [Pg.152]
