Rhinocort Aqua and Nasonex are preparations containing topical nasal corticosteroids (budesonide and mometasone furoate respectively). Otrivine contains a nasal decongestant (xylometazoline) and Sudafed is a systemic preparation containing a nasal decongestant (phenylephrine). Molcer is a preparation for ear-wax removal and which contains docusate sodium. Emadine contains an antihistamine (emedastine) and is presented as eye drops.

Examples of group I, i.e. weak or low efficacy topical steroids, are hydrocortisone acetate in various concentrations, methylprednisolone 1.0% and prednisolone 0.5%. Group II, the moderately potent steroids, includes alclometasone dipropionate 0.05%, hydrocortisone butyrate 0.1%, triamcinolone acetonide 0.025% and fluocinolone ace-tonide 0.01%. Group III, the potent steroids, contains among others betamethasone valerate 0.1%, betamethasone dipropionate 0.05%, budesonide 0.025%, desoximetasone 0.05%, fluticasone propionate 0.05%, amcinonide 0.1%, fluocinonide 0.05% and mometasone furoate 0.1%. Group IV comprises the very potent agents such as clobetasol propionate 0.05% and halobetasol propionate 0.05%.

Beclomethasone dipropionate, and several other glucocorticoids—primarily budesonide and flunisolide and mometasone furoate, administered as aerosols—have been found to be extremely useful in the treatment of asthma (see Chapter 20). 

Beclomethasone dipropionate, triamcinolone acetonide, budesonide, flunisolide, and mometasone furoate are available as nasal sprays for the topical treatment of allergic rhinitis. They are effective at doses (one or two sprays one, two, or three times daily) that in most patients result in plasma levels that are too low to influence adrenal function or have any other systemic effects. 

In this chapter, analysis of the excipient in Nasonex (mometasone naral spray) is used as a typical example to demonstrate how to select a proper analytical method. Nasonex Nasal Spray contains Mometasone Furoate, microcrystalline cellulose, and carboxymethylcellulose sodium NF, citric acid USP, sodium citrate USP, benzalkonium chloride solution NF, glycerin USP, polysorbate 80 NF, phenylethyl alcohol USP, and water USP. 

There are three ways to obtain the total solid (1) determine the water content by Karl Fisher titration, (2) weigh an evaporate portion of product, or (3) sum up all the ingredients, including mometasone furoate and phenylethyl alcohol, which are stated (claimed) on the label. The combination of all three measurements generally gives quite an accurate estimation of the total solid and total cellulose ...

Prednicarbate is a topical glucocorticoid that seems to have an improved benefit-harm balance, as has been shown in 24 healthy volunteers (7 men, 17 women, aged 25-49 years) in a double-blind, randomized, placebo-con-trolled study of the effects of prednicarbate, mometasone furoate, and betamethasone 17-valerate on total skin thickness over 6 weeks (175). On day 36, total skin thickness was reduced by a mean of 1% in test fields treated with vehicle the relative reductions were 13,17, and 24% for prednicarbate, mometasone furoate, and betamethasone 17-valerate respectively. There were visible signs of atrophy or telangiectasia in two subjects each with betamethasone 17-valerate and mometasone furoate, but not with prednicarbate or its vehicle. 

The effect of increasing doses of mometasone furoate and fluticasone propionate by dry powder inhaler on adrenal function was studied by using overnight urinary cortisol in 21 patients with asthma (55). Patients were randomized in a crossover fashion to receive 2-weekly consecutive doubling doses of either fluticasone propionate (500, 1000, and 2000 micrograms/day) or mometasone furoate (400, 800, and 1600 micrograms/day). Beclomethasone dipropionate and several other glucocorticoids—primarily budesonide and flunisolide and mometasone furoate, administered as aerosols—have been found to be effective in the treatment of asthma (see Chapter 20 Drugs Used in Asthma). The switch from therapy with systemic glucocorticoids to aerosol therapy must be undertaken with caution, since manifestations of glucocorticoid deficiency will appear if adrenal function has been suppressed. In such patients, a slow, graded reduction of systemic therapy and monitoring of endogenous adrenal function should accompany the institution of aerosol administration.
