Although topical phenothiazine antihistamine preparations such as promethazine hydrochloride (Phenergan) cream are no longer used in the United States, such topical antihistamine preparations are still widely used in Europe. Individuals who have become sensitized by such topical phenothiazine antihistamines often suffer a flare of the dermatitis when a phenothiazine antihistamine is taken, i.e., a systemic eczematous contact dermatitis. In addition, many individuals acquire allergic sensitization to various phenothiazine drugs which show cross-reactions with the phenothiazine antihistamines. Table 6 lists the phenothiazine antihistamine compounds. 

This antihistamine is used not only in tablet form but also as suppositories. Proctitis and perianal dermatitis have occurred from the use of such suppositories in sensitized individuals. This antihistamine can also produce phototoxic and

PROMETHAZINE HYDROCHLORIDE The preferred parenteral route of administration is deep IM injection properly administered IV doses are well tolerated, but this method is associated with increased hazard. IV administration should not exceed 25 mg/mL at a rate no more than 25 mg/min. Avoid subcutaneous and intra-arterial injection. Use contraindicated in patients younger than 2 years of age. 

Tablets containing chlorpromazine and promethazine were powdered and dissolved in 95% ethanol. The absorbance of a 6 pg/mL solution was measured at 6 wavelengths, and the amount of each component calculated by a least squares method [85]. A spectrophotometric method for the simultaneous determination of 1-18 mg of chlorpromazine hydrochloride and 3-27 mg of promethazine hydrochloride (in 20 mM HCl) was presented, where the absorbance was measured over a spectral region of 275-240 nm [86].

Dual-wavelength spectrophotometric assay was used for the determination of chlorpromazine hydrochloride and promethazine hydrochloride in chlorpromazine hydrochloride composite tablets [91]. Stable regression analysis was used in the simultaneous spectrophotometric determination of chlorpromazine hydrochloride and promethazine hydrochloride in tablets without preliminary separation [92].

Propranolol hydrochloride and promethazine hydrochloride increased the gel points and no straight line relationship existed, indicating that at higher concentrations these drugs enable the polymer to hydrate to a much larger extent than at lower concentrations. Promethazine forms micelles at concentrations. 5% w/v [9], which may be responsible for this behaviour. No critical micellar concentration is known for propanolol hydrochloride but from studies performed it is weakly surface active. The response of HPMC to these drugs may be associated with this surface activity. 

Each teaspoon (5 mL) of Phenergan Syrup Fortis contains 25 mg promethazine hydrochloride in a flavored syrup base with a pH between 5.0 and 5.5 alcohol, 1.5%.
