The existence of chaotic oscillations has been documented in a variety of chemical systems. Key documents that are developed during the planning stages can be used to focus and direct the compliance strategy, to outline the health and safety program/plan requirements, and to establish work controls. These documents are usually developed after contract award and before mobilization. 

A certification document should be developed outlining that the workplace has been evaluated for hazards. It should specify the workplace or areas surveyed and should include the name of the person certifying the evaluation. The contents of a hazard assessment cannot be verified without documentation. Without documentation, the certification could be worthless. So to play it safe, some form of a written certification of the hazard assessment should be retained. 

The standard requires the supplier to prepare a quality manual covering the requirements of the standard and also requires the quality manual to include or make reference to the quality system procedures and outline the structure of the documentation used in the system.

The first document produced under the CCPS program was a brochure entitled "A Challenge to Commitment," which was mailed to the CEOs of more than 1500 companies. It provides an overview and an outline of a comprehensive model for the technical management of chemical process safety, characterized by twelve distinct and essential elements.

Begin by listing all potential causes or combinations of causes, using a brainstorming approach. Then, systematically rule out each cause. Documentation is another essential ingredient in all quality systems. It serves as a mantra for auditors and inspectors that ... if it is not documented it does not exist or it never happened . The documentation requirements can be split into two main categories (i) procedures outlining what must be done and (ii) records showing what has been done.

Palladium-catalyzed aminations of aryl halides is now a well-documented process [86-88], Heo et al. showed that amino-substituted 2-pyridones 54 and 55 can be prepared in a two-step procedure via a microwave-assisted Buchwald-Hartwig amination reaction of 5- or 6-bromo-2-benzyloxypyri-dines 50 and 51 followed by a hydrogenolysis of the benzyl ether 52 and 53, as outlined in Fig. 9 [89]. The first draft is usually based on an existing national, regional, or international review. When no appropriate source document is available, a CICAD may be produced de novo. Whereas ISO Guide 34 sets requirements for the quality system of a GRM producer, ISO Guide 35 (1989) provides guidance on how to implement many of these requirements. Among these, the document also provides a general and statistical outline of the process that leads to CRMs. The current edition of ISO Guide 35 is a little outdated, but stiU most of the contents are valid.
