Certification documents

A certification document should be developed outlining that the workplace has been evaluated for hazards. It should specify the workplace or areas surveyed and should include the name of the person certifying the evaluation. The contents of a hazard assessment cannot be verified without documentation. Without documentation, the certification could be worthless. So to play it safe, some form of a written certification of the hazard assessment should be retained.

It is recommended that Validation Reports follow the same structure as their corresponding Validation Plans so that the two documents can be read side by side and the course of the validation followed step by step. Figure 11.7 illustrates this relationship. A summary for each phase of the validation exercise should be prepared. Details of test outcomes, test certificates, documentation, etc., should be included. Test environments should be described in outline, and any test prerequisites discussed in case they qualify, or even undermine, the overall validation conclusion reached. 

The administrative documentation will be a part of File A (See The Dossier on page 236). The specific certificates/documents required follow. 

Pharmacists who earn a passing score on the NISPC DSM examinations are credentialed and issued a certificate by NISPC. The certificate documents to employers, peers, and the public that they possess the knowledge and skills to provide disease-specific services to patients. NISPC-credentialed pharmacists are recognized on the NISPC and NABP web sites. 

Certification document shows an example of a certificate of completion for a LCV driver training program and states that a certificate is to be issued to a student who completes a LCV driver training program. The certificate issued does not need to match "exactly," but is "substantially in accordance" with the example. Paragraph b of this section creates a requirement that the drivers must provide a copy of the certificate to their employers to file in the driver s qualification file.

At Rovno NPP, three identical sets of pulse safety valves (ISD Sempell type) were installed on the pressurizer of Unit 3. The Rovno NPP considers replacing the existing pressurizer safety valves, since there is no certification document available for the qualification of these valves. The new valves for Unit 4 are of the type UF 50024-100 PS. The certification document of this type of valve indicates that it is used for steam flow, steam-water mixture flow and water flow. But test results provided in the certification only related to air flow and steam flow. 

Independent review of the manufacturing documentation is accomplished through the certification documents. These documents require verification that RTM specifications have been satisfied, process data sheets have been completed and are in place for each subassembly, the final match list is complete, and all NCRs associated with the final assembly have been adequately resolved.
