Planning maintenance

Make maintenance planning more flexible, by providing, wherever possible, viable alternatives to unplanned shutdowns and to immediate component replace or repair actions. Increased flexibility of maintenance planning, by conditionally running under surveillance components with potential or incipient stmctural integrity problems, means improved plant availability, reduced energy replacement and maintenance costs. 

In plant maintenance, the availability of quantitative and non-invasive screening NDT methods will reduce the time needed for shutdowns and increase the intervals between them. Modem NDT methods will become just as important a tool for Risk Based Inspection approaches and maintenance planning as operational parameters and degradation mechanisms already are. 

Conditional Failure Probability. The concept of conditional piobabihty of faiuie is useful to predict the chances of survival for a device that has been in operation for a period of time and is not in a failed state. Such information is helpful for maintenance planning.

If oil is pressurized by compressed air, an explosive hazard exists. If high-pressure air encounters the oil, it may create a diesel effect, which may result in an explosion. A carefully followed preventive maintenance plan is the best precaution against explosions. 

The progress of large shutdowns will be evaluated daily, or as deemed necessary by the maintenance-planning supervisor. This meeting will identify shutdown problems, and see that measures are taken to solve these problems. Also, action will be taken to compensate for off-schedule conditions. The maintenance-planning supervisor should facilitate this meeting. 

Sentinel holes are used as a simple form of thickness testing. A small hole of about I - 6 mm diameter is drilled from the outer wall of the piece of equipment to within a distance from the inner wall (in contact with the corrodent) equal to the corrosion allowance on the equipment (Fig. 9.11). The technique has been used even in cases where the corrodent spontaneously ignites on contact with the atmosphere. The philosophy is that it is better to have a little fire than a big one which would follow a major leak from corrosion through the wall. When the sentinel hole begins to weep fluid a tapered plug is hammered into the hole and remedial maintenance planned. Siting the sentinel holes is somewhat speculative although erosion at the outside of a pipe bend is often monitored in this way.

The operator of the upstream network, the state owned company Gassco AS, is awarded certain powers under Section 66 of the PR. Operational and maintenance plans should be prepared for the computer system and its associated measurement and control instrumentation. Operational plan review will focus on system reliability, performance, diagnostic records, instrument and system I/O calibration, and the provision of critical data to support the batch record. Procedures for controlling the system (e.g., system management, security, and process operations) should be reviewed to verify that they are current, in place, and being followed. For each procedure required for the system there should be documented evidence that the relevant operatives have been trained in its use. All procedures must be written and approved according to the site procedures for writing and approving SOPs. 

The maintenance plan will normally form part of the preventative maintenance system for the site and must be used to track all maintenance activities on the computer system and associated measurement and control instrumentation. For computer systems the supplier may be contracted for different levels of ongoing maintenance support, and it is acceptable to use the supplier procedures for maintenance of the specialist areas of the system. A supplier maintenance contract needs to define the scope of maintenance (e.g., the items to be maintained, type of activities, period of the contract, access requirements,... 

Preventive measures provide conditions where the incident is unlikely to happen, but its occurrence cannot be totally avoided. In this category, we find measures such as inventory reduction for critical substances, the choice of a continuous rather than a batch process leading to smaller reactor volumes, and a semi-batch rather than a full batch process providing additional means of reaction control. Process automation, safety maintenance plans, etc. are also preventative measures. The aim of these measures is to avoid triggering the incident and thus reducing its consequences. In the frame of mnaway risks, a mnaway remains theoretically possible, but due to process control, its severity is limited and the probability of occurrence reduced, such that it can be controlled before it leads to a critical situation. 

The NMED approved an operation and maintenance plan in November 1995, and DBS A started operation of the SVE system in early December 1995. On April 29, 1996, monitor well MW-9 was incorporated into the SVE network to provide further control of the groundwater plume (Fig. 6).

Maintenance Planning can help to minimize the need for contingency procedures by having planned service ontages to maximize the time between failures of the PLC-based system. 

Periodic Reviews are an important element of a continnons improvement program. A topical issne at present is being able to demonstrate that sonrce code has not changed. The use of PLC software tools that enable detailed comparison between software is a useful check during the Periodic Review, and an inspection of Change Control records. Contingency Plans, Maintenance Plans, logbooks, and secnrity measnres demonstrate that the automated system is under control. 

During this period, construction documentation shall be established that provides an accurate basis for system commissioning and validation. Further, operation and maintenance plans, instructions, SOPs, and SLAs shall be developed in readiness for the handover of the assets to the operational environment.
