Documentation sheet of serialized hardness measurements (after Yushkin, 1971)... 

The patient is instructed to record his morning body weight (always under the same conditions) every day on a documentation sheet (E. Kuntz, 1989) as well as the frequency of stools (e.g. under lactulose therapy) and to enter a handwriting specimen (usual way of writing the first and last name) every one or two days. (s. fig. 15.3) This documentation sheet is deemed to be a useful and efficient instrument (which can be kept as a medical record) in the control and follow-up of a chronic course of disease. An ancillary benefit is that the extent to which the patient is cooperating can also be deduced from the accuracy of the entries made. (s. p. 275)... 

Impairment of cerebral functions and disturbances of the water and electrolyte balance are the two most important and most common manifestations of decompensated liver cirrhosis. They may be reliably detected at an early stage by means of daily body weight control and simple psychometric tests. A documentation sheet filled in by the patient has proved to be worthwhile latent oedemas or the onset of ascites as well as latent encephalopathy can be detected in this way and thus treated at an early stage. Longterm standing leads to a reduction of natriuresis with subsequent water retention and a deterioration of renal blood flow (like a vicious circle). This is caused by activation of the RAAS and the sympathetic nervous system. Such a dangerous situation (which can arise for example after two hours of standing at a sports event with excessive emotional participation) is often underrated, as we ourselves observed in several patients (s. p. 292) (s. fig. 15.3) (see chapter 16 )...

Documentation sheet Presentation of the documentation sheet by the patient containing details of body weight and handwriting specimens during the respective follow-up phase. This form (which can easily be drawn up by the patient) may contain, on the front and back, daily entries for 2 follow-up months. This patient-made documentation is the cheapest and most reliable followup method with regard to early detection of the two most sensitive signs of decompensation latent HE and latent ascites, (s. fig. 15.3) (s. pp 275, 305)... 

List of instructions These principles of lifestyle should be closely observed by the patient they apply to every hepatological disease situation. We have always included these principles on a list of instructions given to our patients together with the documentation sheet. (s. fig. 15.3)... 

At this point, we would like to emphasize once again that a strict lactovegetarian protein supply must be adhered to not only in latent encephalopathy, but also in (acute or chronic) liver diseases in which insufficient detoxification of ammonia is anticipated. When assessing the so-called protein tolerance threshold (s. p. 278), the documentation sheet (s. fig. 15.3) as well as a psycho-motoric test programme (s. p. 203) can be of great help. 

After the balance sheet and the income statement or profit-and-loss account, the funds statement is generally regarded as the most important financial document. However, many financial managers regard a statement showing changes in cash as being of equal importance. 

Environmental documentation, including Material Safety Data Sheets (MSDS), military specification data, permits (e.g., NPDES, POTW, RCRA), SARA Title III reports, waste manifests, and any pending permits or application information... 

An overall review of the management of change documentation package should be performed to ensure documentation update items (including material safety data sheets) are addressed and that PHA action items are complete. This can be done by one person but is often best achieved by a PSSR Team. The process needs to account for ... 

It is appropriate to assemble the results in a data sheet as shown in Table 10-3 for planning and documentation. 

The market for PPVC film and sheet is only slightly less than for wire and cable insulation. Uses are as diverse as seepage barriers, factory doors, inflatables, baby pants, car trim, covering materials for book bindings and document cases and shower curtains. UPVC film has also been widely used for packaging of food, particularly for shrink-wrap purposes, but this market has been subject to some substitution by polypropylene. 

MSDS Material Safety Data Sheet - a document that provides pertinent information and a profile of a particular hazardous substance or mixture. An MSDS is normally developed by the manufacturer or formulator of the hazardous substance or mixture. The MSDS is required to be made available to employees and operators whenever there is the likelihood of the hazardous substance or mixture being introduced into the workplace. Some manufacturers prepare MSDS for products that are not considered to be hazardous to show that the product or substance is not hazardous. 

If the control valve size is critical to the overpressure protection of the downstream equipment, and must not be increased, then this is clearly noted in all relevant documentation (specification sheets, flow diagram, operating manual, etc.,) and a warning notice plate is welded to the valve body. In such cases, an actual check of the valve installed or purchased should be made during the startup review. 

Client s Inspector will check, as a minimum, equipment components, dimensions, connection sizes and locations, and quality of workmanship against certified drawings, data sheets, and purchase order documents. 

Complete and accurate written documentation of chemicals properties, process teclinology, and process equipment is essential to the PSM program and to a process hazards analysis (PrHA). This information serves many users including the PrHA team. The needed chemical information includes fire and explosion characteristics, reactivity hazards, safety and health hazards and the corrosion and erosion effects. Current material safety data sheet (MSDS ) information helps meet this requirement, but must be supplemented with process chemistry information regarding runaway reactions, and over-pressure hazards. 

A Change Record, which describes what has been changed. This usually forms part of the document that has been changed and can be either in the form of a box at the side of the sheet (as with drawings) or in the form of a table on a separate sheet (as with specifications). 

Users should be the prime participants in the preparation process so that the resultant documents reflect their needs and are fit for the intended purpose - hence the requirement that documents be reviewed as well as approved. You will need to be able to demonstrate that your documents have in fact been reviewed prior to issue. The presence of a signature on the front cover is not sufficient evidence. To demonstrate that documents have been reviewed you will need to show that nominated personnel have been issued with drafts for comment and that they have provided comments which have been considered by the approval authorities. A simple method is to employ a standard comment sheet on which reviewers can indicate their comments or signify that they have no comment. During the review process you may undertake several revisions. You may feel it necessary to retain these in case of dispute later, but there is no compulsion for you to do so, providing you have evidence that the review took place. You also need to show that the current issue has been reviewed so your comment sheet needs to indicate document issue status. 

There is no simple way of identifying invalid documents, as the reasons that they are invalid will vary. By printing authentic documents on colored paper or providing paper with a special header one can inject a degree of control. Placing the approval signatures on the front sheet will immediately identify an unapproved document. However, the onus must rest with the user who, if properly trained and motivated, will refrain from using invalid documents. 

The change should be processed in the same way as the original document and submitted to the appropriate authorities for approval. If approval is denoted on the front sheet of your documents, you will need to reissue the front sheet with every change. This is another good reason to use separate approval sheets. They save time and paper. 

Many of the documents held at Rapra are not available via other services. This is particularly the case for our extensive and unique collection of company literature and data sheets. 

All formulas are to be written out by hand with the specific numbers in the right places on a sheet provided for the purpose, and the calculations are to be done by calculator. Caution Write the numbers exactly as printed, do not round any digits, or the quality assurance unit (a sort of corporate vice squad) will not approve the report out of fear that someone could have cheated. A validated program can be used. While an Excel spreadsheet as such needs no validation, a simple cell-formula calls for extensive tests and documentation and proof that the sheet is password protected against fraudulent manipulation. On top of that, the analyst s supervisor is required to confirm the calculation and sign off on... 

The revised database holds over 23 000 analyte values for 660 measurands and 1670 reference materials produced by 56 different producers, from 22 countries. The database is restricted to natural matrix materials (i.e. made from naturally occurring materials, excluding calibration standards manufactured from pure chemicals). Information has been extracted from the relevant certificates of analysis, information sheets, and other reports provided by the reference material producers. As a general rule, the authors have only included in the compilation reference materials for which a certificate of analysis or similar documentation is on file. Information included in the survey is on values for measurands determined in reference materials, producers, suppliers, the cost of the materials, the unit size supplied, and the recommended minimum weight of material for analysis, if available. The new searchable database has been designed to help analysts to select reference materials for quality assurance purposes that match as closely as possible, with respect to matrix type and concentrations of the measurands of interest and their samples to be analyzed see Table 8.3. 

Health and Safety Data Sheet This document is required to provide anyone who comes into contact with the material with sufficient information needed to react to a spillage or breakage. The labelling and health and safety sheet regulations are not yet consistent through out the world and at the time of writing there is no way to label a product so that it conforms to US, Canadian, European and Australian requirements The Safety data sheet issue is less confused, the U S OSHA and EU forms of the so called material safety data sheet are moving to conform with an I SO Standard. 

If testing involves a new product that is not yet in commercial trade, the manufacturer should ship products from the same batch of the same formulation to each test location. The most recent product quality analysis and an appropriate Material Safety Data Sheet (MSDS) must accompany the product shipment. The shipping documents and the certificate of analysis for each shipment must be archived in the raw data for the report. If the residue testing is being done to register a new use for a commercial product, the Study Director must make sure that the product available in each location is the same formulation and that each different batch encountered has its own certificate of analysis to be archived along with a copy of the label and product quality analysis of the material used for the test.
