The idea of reduced adrenal capacity as a possible model for PTSD has also been recently raised by Heim et ah, who concluded that low cortisol may not be a unique feature of PTSD, but may represent a more universal phenomenon related to bodily disorders, having an etiology related to chronic stress (Heim et al. 2000). There are numerous stress-related disorders such as chronic fatigue syndrome, fibromyalgia, rheumatoid arthritis, chronic pain syndromes, and other disorders that are characterized by hypocortisolism. In one study, Heim et al. showed decreased cortisol responses to low-dose DEX, but failed to observe blunted ACTH responses to CRF in women with chronic pelvic pain, some of whom had PTSD, compared to women with infertility (Heim et al. 1998). Since the data were not analyzed on the basis of the subgroup with and without trauma and/or PTSD, it is not possible to directly compare results of that study to other reports examining PTSD directly. [Pg.389]

Frequently, MCS involves imbalances in one s nervous, immune, and endocrine systems, and impaired detoxification abilities. In an evaluation of the frequency and clinical characteristics of the underlying connective tissue disorders associated with silicone breast implants, 300 women with silicone breast implants were studied (45). In addition to a history and physical examination, C reactive protein, rheumatoid factor, and autoantibodies were determined. Criteria for fibromyalgia and/or chronic fatigue syndrome were met by 54% connective tissue diseases were detected in 11% and undifferentiated connective tissue disease or human adjuvant disease in 10.6%. A variety of disorders, such as angioedema, frozen shoulder, and a multiple sclerosis-like syndrome, were also found. Several other miscellaneous conditions, including recurrent and unexplained low grade fever, hair loss, skin rash, symptoms of the sicca sjmdrome, Rajmaud s phenomenon, carpal tunnel syndrome, memory loss, headaches, chest pain, and shortness of breath were also seen. Of 93 patients who underwent explantation, 70% reported improvement in their systemic symptoms. [Pg.3139]

Table 18-1 lists the various names given to MCS. The most recent name for MCS is idiopathic environmental intolerances, or IEI (Bornschein et al. 2001). The variety of names reflects the unresolved definition of MCS. A survey of physicians familiar with MCS proposed six characteristics of MCS, as listed in Table 18-2 (Bartha et al. 1999). Table 18-3 lists the most frequent symptoms. MCS symptoms often overlap with symptoms of chronic fatigue syndrome and fibromyalgia (Jason et al. 2000 Nawab et al. 2000). Prevalence of MCS in community surveys and occupational health clinics was found to be 6.3% and 23.0%, respectively (Kreutzer et al. 1999 Kutsogiannis and Davidoff 2001). [Pg.272]
