Successful management

An important initial intervention for a minority of alcohol-dependent patients is the management of alcohol withdrawal through detoxification. The objectives in treating alcohol withdrawal are relief of discomfort, prevention or treatment of complications, and preparation for rehabilitation. Successful management of the alcohol withdrawal syndrome is generally necessary for subsequent efforts at rehabilitation to be successful treatment of withdrawal alone is usually not sufficient, because relapse occurs commonly.

Of even greater importance, the success of any therapy, including pharmacotherapy, depends on the relationship between patient and therapist. The nature and quality of the interaction between the clinician and the patient, flavored by both of their cultural backgrounds, values, attitudes, and expectations, serve as the backdrop against which drugs work, or fail to work. Attention to and successful management of transference and counter-transference are key to the success of not only psychotherapy, but also pharmacotherapy. We did it somehow, almost strangulating ourselves in the process. Damaging fires are uncontrolled chemical reactions, so fire hazards involving ordinary flammable and combustible materials could be included in the above definition of chemical reactivity hazards. However, this publication seeks to supplement basic fire prevention and protection measures by addressing how to successfully manage other chemical reactivity hazards in the work environment. Consequently, the use of the term "chemical reactivity hazards" in this publication will not include explosion, fire and dust explosibility hazards involving the burning of flammable and combustible materials in air. Storage and use of commercial explosives is also outside the scope of this publication. 

Table 2.3 gives one way for an existing facility to get started toward successfully managing chemical reactivity hazards. This assumes you already have an idea as to what chemical reactivity hazards must be addressed by the management system, such as by answering the questions in the Preliminary Screening Method of Chapter 3. 

Table 4.1 lists essential ingredients of a management system for successfully managing chemical reactivity hazards. An honest comparison of a company s current practice with the items in this list can be used to point out the gaps that need to be filled. If you are just getting started with chemical reactivity hazard management, this shows what is needed to develop a successful management system.

Sovner et al. (1998) have done an excellent job summarizing the data on antidepressants in patients with developmental disabilities. There have been nine reports of antidepressant use in adults with depression and MR and three reports of antidepressant use in children and adolescents. Eight of nine reports in adults were positive. The drugs studied included nialimide (n = 27), fluoxetine (9), imipramine (6), amoxapine (2), and nortriptyline (1) (total n = 45). In addition, Sovner et al. identified four reports of antidepressant use in children. One involved successful treatment with fluoxetine in an adolescent, another indicated efficacy with imipramine and amitriptyline in 9 of 12 children (Do-sen, 1982), and a third showed successful management in 3 of 4 children treated with imipramine or tryptophan plus nicotinamide (Dosen, 1990). One study of fluoxetine in depressed children with autism and MR witnessed improvement in depression but not in compulsive symptoms (Ghaziuddin and Tsai, 1991).
