Other failures

Other failures such as tearing, occurring in roundabouts or areas where heavy vehicles brake hard, and tracking, usually long-term failure along the wheel paths, are caused by unsuitability of the surface dressing technique in such sites. The only remedy is to remove the surface dressing and apply an alternative suitable material. [Pg.687]

Scabbing occurs locally owing to inadequate surface preparation and the presence of mud or other contaminants on the existing surface. [Pg.688]

The only remedy is to clean the surface and redress the area. [Pg.688]

In real-life appHcations, many other failure mechanisms are present and this type of curve is not necessarily obtained. For example, in a multicomponents system the quaUty related failures do not necessarily all drop out early but might be phased out over a longer period of time. [Pg.9]

Surface defects, if sufficiently severe, may result in failure by themselves. More commonly, they act as triggering mechanisms for other failure modes. For example, open laps or seams may lead to crevice corrosion or to concentration sites for ions that may induce stress-corrosion cracking. [Pg.316]

Although not commonly listed as a weld defect, high-welding stress nevertheless provides an essential ingredient to stress-corrosion cracking and other failures. It differs in an important respect from other types of weld defects in that stresses cannot be visually identified or revealed by conventional nondestructive testing techniques. [Pg.343]

But what is the proper way to use toughness values The most sensible thing to do is ask suppose the panel is loaded up to its yield load (above this load we knoiu it will begin to fail - by plastic flow - so it does not matter whether other failure mechanisms also appear) what is the maximum crack size that is still stable If this is large enough... [Pg.267]

Below the Chernobyl reactor were water pools meant to capture and condense any steam released from a pipe break or any other failure in the containment rooms. A system of relief valves and ducts led from the containment rooms to these suppression pools. RBMK s were built in pairs ... [Pg.223]

Davenport [1] has listed more than 60 major leaks of flammable materials, most of which resulted in serious fires or unconfined vapor cloud explosions. Table 9-1, derived from his data, classifies the leak by point of origin and shows that pipe failures accounted for half the failures— more than half if we exclude transport containers. It is therefore important to know why pipe failures occur. Following, a number of typical failures (or near failures) are discussed. These and other failures, summarized in References 2 and 3, show that by far the biggest single cause of pipe failures has been the failure of construction teams to follow instructions or to do well what was left to their discretion. The most effective way of reducing pipe failures is to ... [Pg.179]

Many companies now insist that if use of the wrong grade of steel can affect the integrity of the plant, all steel must be checked for composition before use. This applies to flanges, bolts, welding rods, etc., as well as the raw pipe. Steel can be analyzed easily with a spectrographic analyzer. Other failures caused by the use of the wrong construction material are described in Section 16.1. [Pg.192]

After the accident a search disclosed 11 other failures that had occurred but had received little or no publicity [26]. If they had been publicized, this incident could have been avoided. The company concerned withdrew all its carbon dioxide vessels that could not withstand low temperatures and replaced them with stainless steel ones. The company found that it could manage with 75% fewer vessels than it had used before (see Section 21.2.1). [Pg.197]

Active Error/Failure An active human error is an unintended action or an intended action based on a mistaken diagnosis, interpretation, or other failure, which is not recovered and which has significant negative consequences for the system. [Pg.42]

Both time-related failure rates and demand-related failure rates can apply to and be reported for many pieces of equipment. Both types of rates are included in some of the data tables in Chapter 5. If a piece of equipment is in continuous service, such as a transformer, the failure rate is dominated by time-related stresses compared to demand-related stresses. Other failure rates may be dominated by demands. Take a piece of wire and repeatedly bend it. With each bend its probability of catastrophic failure increases. In a relatively short time, if the bending is continued, the wire will fail. On the other hand, the same wire could be installed in a manner that would prevent mechanical bending demands. In this case, the occurrence of catastrophic wire breakage would be remote. In the first instance, the failure rate is dominated by demand stresses and in the second by time-related stresses, such as corrosion. [Pg.7]

Available refrigerants for various levels or conditions of operation may be toxic, flammable, irritating on exposure, hydroscopic, and expensive. These characteristics cannot be ignored, as large systems contain large quantities of refrigerant, and a leak or other failure can release a potentially serious condition into a building or process area. [Pg.290]

Common confusions Vibration analysts often confuse resonance with other failure modes. Since many of the common failure modes tend to create abnormally high vibration levels that appear to be related to a speed change, analysts tend to miss the root cause of these problems. [Pg.742]

One-shot metered lubrication is eminently suited to oiling systems and can be employed either in an all-loss arrangement or as part of a circulatory system. Sight-glasses or other indicators should be incorporated since such lubricating mechanisms are nowadays so reliable that a blockage or other failure might not be suspected until too late. [Pg.884]

Other failures will result from differences in disease etiology, although responders and nonresponders may have the same or very similar symptoms and signs before treatment is started. Some asthmatics do not respond to drugs that block the synthesis of leukotrienes because leukotrienes do not account for their asthma (Drazen et al., 1999). [Pg.167]

Other failures of pipelines generally occur as a result of third party activity and natural hazards. Offshore, pipelines are generally more susceptible to fishing boats dragging their anchors on the seabed. Onshore pipelines are vulnerable to impacts from earth moving operations for construction or road grading. On occasion impacts from mobile equipment may also directly strike and damage the pipeline. [Pg.231]

Franklin Martin, the pioneer of the electrochemical treatment (ECT) of tumors,2 became a major force in the development of surgery in America however, towards the end of his life, he regretted that treatments with electricity had fallen into the hands of quacks and charlatans and therefore into disrepute.7 As was stated in 1991 by Watson 7 Even today this area of research is not favoured by grant-giving bodies and is viewed from the same perspective of disbelief. Also, the lack of easy availability of well-controlled and calibrated power supplies, ammeters and voltmeters—and thus the definition of optimum electrolytic conditions—at the end of nineteenth century, caused electrolytic burns and other failures in some patients so that the technique was never integrated into the routine clinical practice. [Pg.474]

This can be the most important step in the analysis. Understanding the conditions at the site and how the parts or items were used can eliminate some potential failure mechanisms and support other failure mechanisms. Typical questions to ask include ... [Pg.161]

For all other failures, an approved group will decide the correct course of action, for example ... [Pg.26]

At first sight, this poses a fatal dilemma for internal realism. On the first hom of the dilemma we face organizational metaphysical realism. It claims that there are numerous conceptual schemes and they do not simply copy the structure of reality. Such copying would not even be possible, since we do not have direct access to that inherent structure. Rather, the human mind actively projects structures into reality it introduces distinctions, sets up similarity relations, etc. However, there are external constraints on the construction of conceptual schemes, and these constraints are afforded by the inherent structure of reality. Some schemes cut at the joints more often than the others. Failures to cut at the joints are manifested in bad or inaccurate predictions, lack of explanatory power, repeated failures, etc. This view does provide the external constraints necessary for the objectivity of knowledge, and also makes room for the active organizing role of the intellect. But the price is... [Pg.24]

During the discussions of how this specific sequence of events had happened, suggestions were made by operators and engineers that other failure components, which were also considered as feasible, could be added to the Tree, thus adding to the general insight into how this incident and similar ones could be produced", and enlarging the set of related causal factors. [Pg.103]

Much discussion has evolved about the regrettable condition of the railbeds of our country. Derailments occur daily. Much of the rolling stock is also seriously in need of repair. Essentially all tank cars are owned or leased by shippers hence, can be improved at industry expense. Boxcars, gondolas, etc., are usually owned by the railroads (or Conrail) and a broken axle, hotbox or other failure of any car can cause a pile-up, possibly involving tank equipment or trackside hazards. [Pg.267]

A listing of other failures of the unrestricted assumption would be lengthy. Some offer opportunities to the cosmochemist but others thwart the search for the origins of the elements. Examples must suffice. Severe mass loss may expose interior regions previously experiencing nuclear processing (Wolf-Rayet stars, for example) mass transfer across a binary system may expose a stellar core (Algols) or make processed material available in... [Pg.88]

Each of n observations results in only one of two outcomes (one is typically called a success and the other failure). [Pg.61]

A further five, dangerously ill patients were treated between February and June and all but one of these were cured. Their other failure was a four-year-old boy who was seriously ill with infected measles spots. He showed a dramatic recovery with penicillin treatment, and all the staphylococci were eliminated unfortunately, he had suffered from neurological damage during the viral infection, and this was the ultimate cause of death rather than the bacterial infection. [Pg.45]

Using a 10-bit ADC (0-5 V) only 20-26% of the possible range is utilized, which compares with an 8-bit converter. The resulting resolution is 4—7°C per digit. To improve the resolution, additional amplifiers with high requirements for offset and amplification or high-resolution ADCs are required. In these calculations other failures are not included there may be failures caused by resistance of the connection cable or connectors and their thermal properties. [Pg.353]
