Other Reported Incidents in Which Failure to Remove Blinds Created Troubles 

Designers specified a water jacket pressure rating on a small vessel in a soap plant was for a pressure rating lower than the cooling water supply pressure. A simple piping design 

It became necessary to hydrostatically test the vessel jacket, so it was properly blinded and passed the test. But during the startup, as the inlet cooling water valve was opened, the small flat top vessel collapsed. Repairs were expensive and the startup was delayed by several weeks because no one remembered to remove the blind on the valve-free water discharge line. [14] 

In another case, the scrubber that served a storage tank had been isolated to make repairs to the scrubber. Again by an oversight the blind isolating the vent scrubber from the tank was not removed when the scrubber repairs were completed. [14] 

The fiberglass storage tank was overpressured as a transfer was being made into it. An alert operator noticed a remote level indicator seemed to be misbehaving. He wisely
