Mesothelin is a 40-kD glycoprotein of unknown function that is strongly expressed in mesothelial cells, ovarian serous cells, and pancreatic-bile duct cells. Using monoclonal antibody 5B2, Ordonez found it to immu-nostain normal mesothelial cells, mesotheliomas, non-mucinous ovarian carcinomas, and occasionally other neoplasms. Ordonez concluded that mesothelin staining could be used to diagnose mesotheliomas, although it was expressed in 14 of 14 ovarian carcinomas, 12 of 14 pancreatic ductal adenocarcinomas, 7 of 12 desmoplastic small round cell tumors, and 9 of 9 synovial sarcomas. Therefore, this antibody should be interpreted carefully. 

D2-40, a clone of podoplanin, is a recently developed commercially available antibody directed against the M2A antigen, a 40,000-kD sialoglycoprotein associated with germ cells and lymphatic endothelium. Chu and colleagues evaluated 53 cases of mesothelioma, 28 cases of reactive pleural tissue, 30 cases of pulmonary adenocarcinoma, 35 cases of renal cell carcinoma, 26 cases of ovarian serous carcinoma, 16 cases of invasive breast carcinoma, 11 cases of prostatic adenocarcinoma, and 7 cases of urothelial carcinoma. The authors found D2-40 expression in 51 of 53 (96%) mesotheliomas, 27 of 28 (96%) reactive pleural tissues, and 17 of 26 (65%) ovarian serous carcinomas. They did not find D2-40 in the other tumors examined. The authors also observed that the neoplastic cells immunostained in a cell membrane distribution.

Einally, it should be noted that while many serous adenocarcinomas of endometrium resemble ovarian serous carcinomas, there are some important differences. We will discuss these differences in the Ovary section. The most important of these is infrequent WTl expression in endometrial serous carcinomas (seen in at most 20% to 30% of such cases) and the very common diffuse nuclear expression of WTI in ovarian, tubal, and primary peritoneal examples (at least 70% to 80% of such cases).

Wilms tumor gene product (WTl) has been established as a good marker for mesothelial cells. Studies have also demonstrated that WTl is a highly sensitive and specific marker of serous carcinomas of ovarian surface epithelial origin (both ovarian and extra-ovarian). Recently, Caduff et al. (1999) have evaluated abnormalities in p53 and Ki-ra.v in malignant and borderline ovarian tumors of various histological types in paraffin-embedded tissues. The patterns of these genetic alterations in borderline and malignant neoplasms were compared and correlated with cell type and stage. This preliminary molecular analysis suggests that serous borderline tumors have the same molecular features usually associated with malignancy but are unlikely to represent a precursor of invasive serous carcinoma. On the other hand, mucinous borderline tumors may represent a precursor or variant of mucinous carcinoma of the ovary.

A second, independent study repeated the visual inspection analysis and compared the sensitivities and specificities obtained using visual inspection and a precursor of the SCS analysis on the same data set.43 Biopsy samples of postmenopausal ovaries, benign ovarian neoplasms and ovarian cancers ( 0.5 g) were obtained during surgery. The histological types of these samples were normal (19), benign tumour (3), borderline tumour (3), serous carcinoma (13), endometroid carcinoma (17), mixed epithelial carcinoma (3), and small cell carcinoma (4). FR expression may also be dependent on the histological classification of a tissue or cancer. For example, using a recently developed quantitative assay for measuring functional FR in cells and tissues, it was found that normal ovarian tissue and the mucinous form of ovarian cancer express very low levels of FR ( 1 pmol FR/ mg protein unpublished data, Endocyte, Inc., West Lafayette, IN). However, serous ovarian carcinomas express high amounts of FR, with the average expression level being 30 pmol FR/mg protein, or about 30-fold higher than that of the normal...
