The number of immunocompromised persons is continually increasing as advances are made in medicine. The life expectancy for persons with cancer, HIV infection, and solid organ or bone marrow transplantation is increasing. Vaccination provides one tool to prevent infection in the immunocompromised host however, the individual s immuno-suppressed state will alter the response to vaccine. In general all vaccinations should be updated prior to the person becoming immunosuppressed, if possible. Once a person becomes significantly immunosuppressed, live virus vaccines should be avoided.

Finally, we consider physiological expression or suppression, and the long latent period between carcinogen treatment and tumor appearance. While it makes sense to think in terms of phenotypic lag, clonal growth, vascularization, immuno-suppression, and other phenomena, we just do not know enough today to define clear-cut experimental approaches. However, at the human level, the undesirable endpoints of this series of processes are clear teratologic syndromes, cancer, and heritable birth defects. 

There are occasional anomalies to the rule that food reduces and delays peak plasma concentration. The anti-fungal drug, griseofulvin, has enhanced absorption if taken with a meal - possibly because it becomes emulsified by bile salts and passes more readily into the lymphatic drainage of the gut which bypasses the liver, entering the venous system directly. In severe acute disease parenteral steroids and other immuno-suppressants, ciclosporin, and infliximab may be required. There is little evidence to support the use of any accessory treatments. Constipation associated with colonic dysfunction may be helped by a high fibre diet but this will have no intrinsic effect on the inflammation. A series of mass mortalities in aquatic mammals occurred in the last decade. An important advance in preclinical models for anticancer agents was the development of immuno-suppressed mouse strains that allowed for the reproducible implantation and growth of human tumor cells in vivo. Resveratrol has been reported to have chemo-preventive activity in a mouse model of carcinogenesis, but there is a concentration-dependent effect, and at the concentrations used in vitro the effect is predominantly antiproliferative. It has also been reported to have mitochondrial-dependent apoptotic activity against a variety of cell lines. Resveratrol has been reported to inhibit the constitutive COX-1, but it is not clear whether it inhibits the inducible COX-2. These cyclo-oxygenases stimulate tumor growth by acting on cell proliferation, angiogenesis, and immuno-suppression.f °° ... 

Liposomal amphotericin has been given to an immuno-suppressed renal transplant patient with cerebral aspergillosis for almost 10 months at a cumulative dose of 42 g with no apparent changes in the function of the renal... 

The indications for daclizumab arc prophylaxis of acute organ rejection in patients receiving renal tran.splants, as part of an immuno.suppressant regimen including cyclosporine and corticosteroids. The mechanism of action is the same as that of basiliximab.
