Observers properly trained

Observations must be public in that they must be replicable by any properly trained observer. The experienced conditions that led to the report of certain experiences must be described in sufficient detail that others can duplicate them and consequently have experiences that meet criteria of identically. That someone else may set up similar conditions but not have the same experiences proves that the original investigator gave an incorrect description of the conditions and observations, or that he was not aware of certain essential aspects of the conditions. 

Use the tool correctly. Just because a tool meets all the theoretical requirements does not mean it will operate properly under adverse conditions or that it will be used correctly by those not properly trained in its operation. Observe the tool in action.

A trained operator then executes the protocols. If events or data that do not match the expected results are observed, then an error log must be written. The error log details the issue and its resolution. Proper retesting of a failed protocol can then occur. Following completion of the execution of the protocols, customer signoff is again required. 

Closely related to preventive action is the process audit. Such an audit is performed as part of a laboratory assessment for each test on the proposed scope of accreditation.
