The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR American Psychiatric Association, 2000) specifies that the symptoms of substance-related disorders may include tolerance withdrawal loss of control unsuccessful efforts to cut down or quit a great deal of time committed to finding, using, or recovering from using substances impairment in specific areas of one s life and continued use in spite of negative consequences. To meet criteria for dependence, the individual must have three or more of these... 

A person can meet criteria for abuse or dependence for one drug and not others, although there is concern about what has been referred to as cross-addiction. Cross-addiction means that a person who is dependent upon one substance may be dependent upon another, very similar one. An example might be a person who is diagnosed as dependent upon tranquilizers and who, you may fear, is also addicted to a similar substance such as alcohol. However, the research is not entirely clear on whether cross-addiction occurs, and I have known clients who had very specific problems with one substance who did not generalize into problems with other, similar substances. A person also may meet criteria for abuse or dependence for multiple substances generally, this is referred to more simply as substance abuse or substance dependence. 

Antisocial Personality Disorder may be co-occurring with the drug use. Many more men meet criteria for this disorder than women, but there are women who do meet these criteria. Sometimes borderline (described later in this section) behavior can be mistaken for antisocial, and certainly there can be overlap between the two disorders. However, clients with whom I ve worked who have borderline features generally have the capacity to care about other people, whereas people who are antisocial may not have this capacity. 

Professionals working with a suicidal drug client may wish to determine whether the person meets criteria for Borderline Personality Disorder. Borderline clients often have a history of suicidal behavior and high utilization of health and mental health care services. Most people who meet criteria for Borderline Personality Disorder are women, but not all. As mentioned, some professionals find it difficult to work with borderline clients without becoming very upset or cynical. If you cannot work with such a client respectfully, then it is recommended that a referral be made to someone who can (see Chapter 3). Treating the client with dignity is important if trust and a solid therapeutic alliance are to develop.

In adults with OCD there appears to be an overlap with eating disorders (Pigott et ah, 1994), but this is much less common in pediatric samples. On occasion, youth may meet criteria for both OCD and another OCD-related diagnosis.

Children and adults with PTSD commonly meet criteria for other psychiatric disorders (Breslau et ah, 1991 Goenjian et ah, 1995 Brady, 1997 De Beilis, 1997). In the adult PTSD literature, comorbidity is clearly the rule rather than the exception and multiple comorbidities are the rule within the rule. Kessler et al. (1995) provide data from interviews with over 6000 individuals ages 15-54 in the National Comorbidity Survey indicating that 88% of men and 79% of women with PTSD had at least one comorbid disorder. Affective disorders, anxiety disorders, and substance use disorders are the most common comorbid conditions in individuals with PTSD (Kessler et ah, 1995 Brady, 1997 Solomon and Bleich, 1998). 

Clinical documentation of sensitization, or kindling, patterns in humans is difficult to compile, but Post and colleagues have noted that traumatic life events become less evident with incremental episodes. Sixty percent of the patients who have symptoms that meet criteria for their first episode of MDD have identifiable precipitating events, as contrasted with 36% of those with two or more episodes (Post 1994).
