Documentation operation manual

Documentation Operating manuals, service procedures and schedule, cost and benefit analysis, analyzer history, service records (based on control charts), drawings, etc. should be well documented and organized. 

Environmental awareness is a prime concern in all KOH plants. Safety precautions required in KOH and chlorine operations are well documented in operating manuals and sales brochures pubUshed by all commercial producers. Discharges of waste effluents containing mercury are strictly forbidden. 

If the control valve size is critical to the overpressure protection of the downstream equipment, and must not be increased, then this is clearly noted in all relevant documentation (specification sheets, flow diagram, operating manual, etc.,) and a warning notice plate is welded to the valve body. In such cases, an actual check of the valve installed or purchased should be made during the startup review. 

In addition to the documents that serve product manufacture and installation or service delivery, documents may also be required for maintenance and operation. The product descriptions, handbooks, operating manuals, user guides, and other documents which support the product or service in use are as much a part of the design as the other product requirements. Unlike the manufacturing data, the support documents may be published either generally or supplied with the product to the customer. The design of such documentation is critical to the success of the product, as poorly constructed handbooks can be detrimental to sales. 

Documents such as job descriptions, operating manuals, emergency procedures, accident, and "near-accident" records, can be useful sources of information about the task to be studied. Pipework and instrumentation diagrams can also be used to gain an insight into the complexity of the process, the type of control loops installed, and the process parameters to be manually controlled by the workers. 

The project may or may not be complete when results are delivered to the client. Often there are documentation requirements such as operation manuals, complete...

ERPs do not necessarily need to be one document. They may consist of an overview document, individual emergency action procedures, checklists, additions to existing operations manuals, appendices, etc. There may be separate, more detailed plans for specific incidents. There may be plans that do not include particularly sensitive information and those that do. Existing applicable documents should be referenced in the ERP (e.g., chemical Risk Management Program, contamination response). 

Documentation Validation documents are current and the requirements are being met by the instrument, SOPs, operation manuals, business continuity plan, and location of the documents. 

Check the documentation for completeness (operating manuals, maintenance instructions, standard operating procedures for testing, safety, and validation certificates). 

Documentation of process procedures in a manual or set of guidelines ensures that job duties are precisely defined. This is especially important in an integrated production process where unsatisfactory performance during one step can adversely affect the remaining steps of the process. A control plan is the sum of the procedures and equipment used to ensure that the internal requirements are met. It includes control charts, sampling plans, 100% inspection, feed-forward/feedback mechanisms, and mistake-proofing tech-niques/devices. Some items may be performed by equipment like automatic controllers, and some are performed by operators and checkers. There may not be a single document called a control plan. Instead, these controls might be spread across a number of documents including a statistical process control plan, an inspection plan, an operator manual, and various other standard operating procedure and specifications. 

Change control of the ERP hardware, software and associated documentation (SOPs and operating manuals) is necessary and requires that changes be recorded, specified and assessed for GxP impact. Changes are to be approved by qualified personnel and tested, with records of testing retained.

In the specific implementation of a safety audit project, safety audit project leader should be fully aware of the following circumstances, an initid assessment of the importance of this safety audit and audit risk (1) ask the audited entity related personnel and related management (2) access to the audited entity s operations manual, the internal control manual and other information (3) access to the audited entity annual production and safety plans, financial budget and other documents (4) check the safety activities or safety issues certificates and records (5) examine the audited unit production, business activities and the implementation of internal control (6) select the number of safety events to be tested. 

Many operating manuals are cocooned in a cloud of soft materials such as introductions, summaries, generic safety instructions, and company mission statements. None of these materials help the operator do his or her work more efficiently or more safely instead they pad the manual with material that gets in the way, thereby increasing the time it takes find the pertinent instructions. In response to this concern, the minimalist approach eliminates these soft materials. An operating manual should tell the operator how to operate the unit—no more, no less. AU other material should be located in other documents. 

In order to improve on this situation, the writers of operating manuals must understand that their competition is not the chemical facility, refinery, or gas facility down the road. Their competition is the world of attractive and appealing published documents available to the public. 

The internal norms may surface in internal procedures, technical project specifications, operating manuals and the like. These documents will in turn constitute relevant objects for state safety control state supervisory bodies evaluate the solutions and may react by requiring amendments to be made. More likely, the choices made by industry will be reviewed by the supervisory bodies in the context of the industry s applications for the various approvals, permits and licences needed to conduct the petroleum activities. 

HCF procedure development maintains consistency by follow/ing the TA-V Nuclear Facilities Conduct of Operations Manual (SNL 1998a) and the SNL ES H Manual (SNL 1998b). These documents describe procedure format and content, including Purpose, Scope, Ownership, Responsibilities, Definitions and Acronyms, Hazard Identification, Equipment and Materials, Format, Review and Approval Authority, and Document Control. Use of this format complies with DOE Order 5480.19, Conduct of Operations, Chapter 16 (DOE 1990). TA-V Standard Operating Procedures are written for tasks specifically identified in the Technical Safely Requirements (TSR) or as required by other directives or the SNL ES H manual to address special hazards. TA-V document types and hierarchy is described in TA-V Nuclear Facilities Conduct of Operations Manual (SNL 1998a) Chapter 16.
