The National Transportation Safety Board (NTSB) has found much evidence to indicate the probable cause of the January 6, 2005, collision and derailment of Norfolk Southern train 192 in Graniteville, South Carolina, was the failure of the crew of Norfolk Southern train P22 to return the main line switch to the normal position after the crew completed work at an industry track. Contributing to the severity of the accident was the puncture of the ninth car in the train, a tank car containing chlorine, which resulted in the release of poison gas.

This section is being added as a result of a National Transportation Safety Board (NTSB) investigation of a truck crash in Pennsylvania. The NTSB discovered that driver training in the areas of air brake inspection and repair was insufficient in the drivers that had driven the truck. Whenever the slack adjusters on the truck had been found to be out of adjustment, the slack adjusters were simple reset. No one attempted to find the cause of the problem (the brakes being constantly out of adjustment).

The US National Transportation Safety Board (NTSB) issued various Safety Recommendations on May 31, 1996. These included a recommendation that the Boeing 757 flight manual should be revised to notify pilots that Simultaneous activation of the mach speed trim and rudder ratio advisories is an indication of an airspeed discrepancy. The NTSB also required Boeing to modify the alarm system to include a caution alert when an erroneous airspeed indication is selected. Various other changes to the flight manual were also instructed. Occurrence of a change can affect safety performance of a system. In order to prevent them, risks analyses are conducted. In some industiy like train transportation, studies is mandated by a regulation, in other industries studies are voluntary done. 

The dismal results of past evaluation studies of driver education led the US National Transportation Safety Board to conclude in 2005 that although driver education has been available since the 1930s and, intuitively, should improve driving safety, in fact little consensus exists on the benefits of driver education and training, what it should entail, and how it should be delivered. (NTSB, 2005). There is obviously a need for a new approach to driver education. The GDL, as comprehensive as it may be, still must rely on some driver education. While it has provided insights into the importance of the component phases in the training, it has not specified the specific contents that the novice driver must learn and internalize.

The National Transportation Safety Board (NTSB) is calling for improvements in cargo tank design and in driver training, after identifying both as factors in a 2009 Indianapolis truck rollover accident that injured five people, two seriously. 

This particular book deals with the development of motor fleet transportation safety and identifies problems that are inherent among security personnel. Two studies give some insights into whom trespassers are, and the circumstances and locations in which they are struck by trains. One is a National Transportation Safety Board (NTSB, 1978a) study of 280 fatalities that occurred between March 1976 and October 1977. The other examined coroners reports for all of the 138 trespasser deaths in North Carolina for the years 1990-94 (Pelletier, 1997). The results of the two studies are almost identical.
