Servicing instructions

The following quality standards for cooling-pond water are prescribed by a special service instruction ... 

Maintenance spares, special handling practices and tools, cleaning, media backups and restoration, service contracts, service instructions, calibration schedules... 

The basis for maintenance work should of course be provided by the operating and servicing instructions issued by the suppliers of the various items of plant and equipment. 

It is recommended that the following general procednres be observed by qnaUfied personnel in the inspection and repair of motor controller involved in a fanlt. Mannfacturer s service instructions should be consulted for additional details.

Produce procedures that govern the generation, proving, and publication of product/ service support documents, such as handbooks, operating instructions, etc. 

The standard requires controlled conditions to include documented procedures defining the manner of production, installation, and servicing where the absence of such instructions would adversely affect quality. The supplementary requirements also require job instructions for all employees having responsibilities for the operation of processes and for these job instructions to be accessible for use at the work station without disruption to the job. 

Define service restoration instructions covering the actions required to restore equipment or facilities into service, including restoration and response times. This is usually first line maintenance and may not require any repair action. 

Provide servicing staff with current instruction manuals for the equipment they are servicing. 

Do provide staff with adequate instructions on how to carry out the servicing. 

There should be a formal handover from construction so that everyone is aware when it has taken place. The final connection to process or service lines is best made by plant fitters rather than by the construction team. In each plant, the procedure for handover should be described in a plant instruction. 

The root causes for this critical event both concern the operations department and the service department who ran the contractor maintenance team. The operations department (i.e., the day shift operations supervisor) failed to provide adequate supervision and instructions to the contractor team. Explanations of the nature of the permit-to-work systems (i.e., the need to report back at end of shift) should have been given, and the possibility and implications of work not being completed before the end of the shift should have been considered by both parties.

The manufacturer of structures that satisfy API Standard 4F must also furnish the purchaser with one set of instructions that covers operational features, block reeving diagram, and lubrication points for each drilling or well servicing structure. Instructions should include the raising and lowering of the mast and a facsimile of the API nameplate. 

These general recommendations, if followed, should result in longer satisfactory service from the equipment. These recommendations should in every case be considered as supplemental to, and not as a substitute for, the manufacturer s instructions. 

The drilling and well servicing structure manufacturer has carefully designed and selected materials for his or her portable mast. The mast should perform satisfactorily within the stipulated load capacities and in accordance with the instructions. Every operator should study the instructions and be prepared for erecting, lowering, and using the mast. 

If the purchaser is not satisfied with the wire rope service, he or she shall send the properly preserved sample or a sample of the rope from an unused section to any testing laboratory mutually agreed upon by the purchaser and the manufacturer, with instructions to make a complete API test, and notify the manufacturer to have a representative present. If the report indicates compliance with specifications, the purchaser shall assume cost of testing otherwise, the manufacturer shall assume the expense and make satisfactory adjustments not exceeding full purchase price of the rope. If the report indicates noncompliance with specifications, the testing laboratory shall forward a copy of the test report to the manufacturer. 

The guidance notes can be produced either as a composite handbook containing task instmctions for all types of plant and equipment for each trade group, or as specific task/advice notes for each asset of the service requirements of the plant equipment. When used, the handbook method provides the necessary work instructions for all similar types of plant and equipment throughout the location. This method of operation reduces the number of task/advice notes issued and therefore the system s workload, as well as the demands placed on the administration of the system. Either method eliminates the requirement of entering work instmctions, etc. on the work docket/advice notes before they are issued. 

Most oil companies offer a planned lubrication maintenance (PLM) service that will meet these requirements with the minimum of effort on the part of the customer. These schemes provide logical routing for the lubrication operative, balanced workloads and clear instructions to those responsible for specific tasks associated with lubrication and fault-reporting facilities. Many schemes are now designed for computer operation, which also accommodate plant and grade changes, operation costs and manpower planning. It is essential that any such scheme should be adaptable to individual requirements. 

It is not proposed here to provide instruction in the techniques of standard costing but merely to illustrate the uses to which certain of the methods can be put. This can best be shown by the use of a worked example. The following is the statement for a service department supplying a factory, which uses both fuel and materials. 

The situation often arises where equipment is brought into service before it has been commissioned and accepted by the purchaser. Such operation can only be on the instructions of the purchaser and with the agreement of the supplier. A maintenance contract must come into force at this time. 

After obtaining the prototypes, tests must be made to determine the utility. Generally these include a short time destructive test to determine the strength and to check out the basic design. Another test that is done is to use the product in the projected environment with stress levels increased in a rational manner to make for an accelerated life test. Other tests may include consumer acceptance tests to determine what instructions in proper use are required, tests for potential safety hazards, electrical tests, self-extinguishing tests, and any others that the product requires. In the case of high risk products, the test program is continued even after the product enters service. 

Installation qualification involves performing checks to ensure that the correct equipment or system has been installed and/or connected, including all necessary controls, monitors, instrumentation, or ancillary services. These checks should include verification that relevant operator manuals or instructions have been received from the supplier and that any applicable calibration steps have been identified.

Every part of an emergency plan has to be understood by all personnel involved, including the emergency services. Detailed instructions therefore have to be issued to all those who will act to control, or mitigate the effects of, a major incident. Defined maintenance procedures should include model or manufacturer specifics and a list of parts to be inspected, cleaned, lubricated, replaced, and/or calibrated. The replacement part numbers, cleaning solutions and lubricants, and calibration standards, along with the manufacturer s maintenance procedures to be followed, should be specified. Documentation is easily managed by creating a one-page checklist of instructions or performance parameters that can be checked off as each task is completed. Any issues or comments can be captured directly on the checklist. Provisions for failure or out-of-tolerance notification need to be clearly defined, as the equipment cannot be put back into service until the performance has been verified by conducting a performance qualification. 

The collaboration between mental health services and members of families of people with mental illness and their other carers is of essential importance in developing a useful and successful mental health service. Collaboration with carers is not in existence in many of the Third World countries, and in places where it exists it is at present often restricted to a one-way communication of instruction of what the carers should do to assist in the realization of the treatment plan established by the medical practitioners. Advice from carers and information about the effects of treatment can both be of great value in the treatment of individual patients and in deciding the best use of medications and other treatments in health services in general.
