The Center for Chemical Process Safety (CCPS) has identified the need for a publication dealing with process safety issues unique to batch reaction systems. This book, Guidelines for Process Safety in Batch Reaction Systems, attempts to aid in the safe design, operation and maintenance of batch and semi-batch reaction systems. Provide a how-to guide for the practicing engineer to identify, define, and address unique safety issues typically encountered in batch reaction systems. 

Each chapter starts with a description of the topic covered in the chapter. This is followed by a short example highlighting a reported incident involving a batch reaction system. The case study is followed by a listing of key issues and process safety practices unique to the topic. The issues and concerns presented in this book, as well as potential design solutions and sources of additional information are presented in the tables. This format concisely conveys the necessary and relevant information in a familiar and convenient format. The organization of the tables is described below. 

Using a standard audit form is certainly acceptable and can prove to be an outstanding tool, especially when getting a handle on a fire extinguisher program, life-safety issues, the lighting of exit signs, availability of first-aid kits, clear aisles, and a variety of issues that are basic in nature. There are several important questions in drug safety evaluation How to detect unexpected adverse drug reactions while handling the multiplicity issue properly How to s)mthesize data from different trials, or even different sources How to deal with rare events How to evaluate multidimensional, complex safety information as a whole Can we monitor a potential safety issue in a continuous manner during a trial so that patients can be better protected These questions lead to some unique statistical challenges in quantitative safety analysis including low power due to rare events, multiplicity. 

Conventional industrial hazards involve other kinds of safety issues, which are not unique to a nuclear facility. An example of conventional industrial hazard is an operator falling from a ladder. It should be noted that in some cases, industrial hazards receive less attention and thus cause more problems. Because of the time limit, the details will not be discussed here. 

In the previous two chapters it was noted that offshore safety management has much in common with other process industries, particularly onshore facilities such as refineries, pipelines, and chemical plants. However, each industry does have its unique safety issues and problems. During the operational phase, it is necessary to ensure that the functional safety of the computer based system is maintained both during and after modifications. As mentioned in para. 4.24, adequate change control procedures which deal specifically with plant safety issues should therefore be in place. It is also important that the safety impact of the modifications be analysed by personnel from appropriate disciplines. In this respect, modifications to computer based systems are no different from other plant modifications. However, there are certain specific issues that apply uniquely to computer based systems and their software, and these are addressed in this section. 

CO methanation is an effective means of removing trace CO in a hydrogen steam. The process is technically simple because it does not require the introduction of any gases, such as air, as is required in CO preferential oxidation. The process is normally operated at temperatures from 180 to 280°C [47]. However, CO methanation is not suitable for an onboard application as compared to preferential oxidation of CO, which requires temperatures of only 80 to 180°C. The hazards associated with continuous haulage systems underground have been described by el-Bassioni (1996) in an article that documents fatalities, injuries, safety issues, and safe work procedures. Continuous haulage systems, also known as bridge conveyors, are used with continuous miners. Concerns unique to continuous haulage systems include lack of communication between a miner and haulage operators, the lack of space around the systems, and Hmited visibiUty. Figure 26.6 illustrates an actual fatality in a coal mine. The operator was killed when his shuttle car bumped into the rib, and he was crushed between the rib and the car. 

This book is intended to fill the gap between a technical manual and the many topic guidance notes such as those published by the Health and Safety Executive. Uniquely, the amount of space devoted to construction EH S management is the same as that given over to techniques and to the law. Experience shows that time spent on pre-construction activity and planning Is amply repaid during the construction phase of work, when safety Issues can be much more difficult to resolve.
