Audits forms

The audit/inspeetion form that you should use ean be developed from the safety plan. A qualified person should examine the safety plan and eome up with a eheeklist that should serve as an audit/inspeetion form. Allowanees should be made to inelude items not speeifieally noted in the safety plan but that may be observed during field walk-throughs. Certain highly pertinent seetions of what OSHA uses when performing a eomplianee inspeetion of hazardous waste sites is ineluded in Appendix D. This inspeetion/audit form eovers many of the basies and ean be used a general guide. 

Using a standard audit form is certainly acceptable and can prove to be an outstanding tool, especially when getting a handle on a fire extinguisher program, life-safety issues, the lighting of exit signs, availability of first-aid kits, clear aisles, and a variety of issues that are basic in nature. The typical standard audit form will likely have complete sections that are marked not applicable. Unless a unique audit form is created, there is usually no way around sections that will be marked not applicable. ...

It should be appreciated that a waste audit forms the vital activity of data col-lechon and investigation of the current condition of the factory and its activities. Therefore, it is the very base of the next series of activities. The waste audit should be undertaken with utmost care and should be as thorough as possible. The more effort is spend at this phase, the better will be the chance of success of the project. 

Audit forms will usually contain four major sections ... 

The quality audit is a communication device. The audit form is given to suppliers in advance. And the form includes descriptions of how each item... 

The Worksheet is, in a sense, an audit form. As will be shown in Chapter 21, ZIO, Other Safety Standards and Guidelines, and VPP Certification, the VPP safety management system requirements have great similarity with the ZIO provisions. However, there are differences. Some of the VPP requirements are not as specific as comparable provisions in ZIO. Those provisions are identified below along with cross-references to the relevant chapter in this text ...

ANSI Z9.5-2003, Laboratory Ventilation, is intended for use by employers, architects, occupational and environmental health and safety professionals, and others concerned with the control of exposure to airborne contaminants. The book includes new chapters on performance tests, air cleaning, preventative maintenance, and work practices. It also highlights the standard s requirements and offers good practices for laboratories to follow. The book also offers referenced standards and publications, guidance on selecting laboratory stack designs, an audit form for ANSI Z9.5, and a sample table of contents for a laboratory ventilation management plan. 

The auditor shall write his questions on the left hand side of the audit form Figure 9.4 (2/3), but shall only reveal them to the auditee during the audit. 

A standard audit form should be developed and used by all divisions or departments. This form must be clearly understood by all auditing teams and strictly adhered to as the standard tool. An example of such an audit form is shown in Figure 11.1. 

Team size and frequency will depend on the size of the property. At the beginning of every shift, employees should inspect their work areas and equipment before starting work to ensure that all defects are noted and corrected. Figure 11.2 is a sample daily audit form. The safety reminder checklist is not a complete list of safety topics and should be structured for your work area or fadhty. Note that the items listed in Figure 11.2arenot only positive work practices, but also government requirements.

The use of workplace inspections makes it necessary to have or develop written safety and health audit instruments. These instruments need to be tailored to meet the specific needs and intended purposes. Those using audit instruments need to be trained on their use and application. This means that they should be able to identify the hazards that are recorded on the audit form and must understand the workings of the workplace, process, equipment, etc., that they are to audit. These completed instruments are a record and documentation of one facet of your safety and health effort. A written sample safety and health audit policy is found in Figure 7.4. In Figure 7.5, you can find a sample jobsite inspection that can be altered and tailored to meet the unique needs of a particular company. 

Placing too much emphasis on injury-producing events but not focusing on potentially serious close call incidents can result in unreliable effective assessments. Rather than relying solely on injury rates or other postevent assessments, organizations could use a broader hazard control audit process. This management style audit would address several key components of the accident prevention process. The audit forms would help evaluators rate each component against prepublished... 

In many ways, environmental audits are the easiest and most acceptable type of evaluation. In fact, regular environmental or housekeeping audits are already standard practice at most companies. These evaluations can often be improved by involving more employees in designing audit forms, conducting systematic and regular assessments, and posting the results in relevant work areas. 

The other operation was called repackaging. This operation received bulk chemicals in tank wagons or rail tank cars and either placed the chemicals in a storage tank or immediately drummed them into smaller containers. Whatever was repackaged would be placed into storage and distributed using the distribution operation. Both of these operations were under my supervision and shared the same physical location therefore, I was tasked by corporate management to perform the standard corporate safety audit on these two different operations. With the diversity between the two divisions, the use of one audit form just did not seem to fit. 

In retrospect, this company should have customized the audit form for each operation that would have fit the particular safety concerns of that operation. The corporate management did not want to deal with different audit forms they wanted consistency. They believed that using one audit form was the fair way to judge one facility against the other. What might have been an alternative would be to offer one type of audit for the repackaging operations and another audit for warehousing/distribution operations. The comparison then would have been closer to the apples to apples comparison.
