SAFETY ASSURANCE PROGRAMS

This is the latest volume in a series on ammonia plant safety. These technical manuals give guidance on safety development, accident prevention, and efficient plant operation. Safety Constraint SMC must ensure that the prime contractor creates an effective development and safety assurance program.

The OSHA standard is required by the Clean Air Aet Amendments, as is the Environmental Proteetion Ageney s Risk Management Plan, whieh was proposed in 1992. Employers who merge the two sets of requirements into their proeess safety management program will better assure full eomplianee with eaeh as well as enhanee their relationship with the loeal eommunity. 

Dairy Quality Assurance Program. This program attempts to learn exactly what the producer needs in terms of technology, management practices, training, recordkeeping, equipment, veterinary care, and any other parts of the production business that contribute to the ability to market the product from the perspective of food quality and safety. 

In contrast, ACR emphasizes the quality control, calibration of the equipment, and technology used in the procedures, and accordingly has developed three modules - module 1 for oncology, module 2 for brain, and module 3 for heart. A PET center must apply for all modules that are performed at the facility. For accreditation, the facility is required to submit information on the quality control and quality assurance program, data collection, reporting, radiopharmaceuticals procedures, and laboratory safety, along with chnical and approved phantom images. So, all equipment in a PET center is required to be calibrated and tested for accurate functionahty for accreditation by ACR.

Initial startup testing procedures have been prepared and implemented to demonstrate that structures, systems, and components (SSCs) and processes will perform as intended. Initial testing includes, as appropriate, bench tests and proof tests prior to installation, mockup tests, pre-operational tests, post-maintenance tests, post-modification tests, and operational startup tests. Safety-related items are subject to the quality-assurance requirements of SNL/NM Research Reactor and Experimental Programs (RREP) Quality Assurance Program Plan (SNL 1998a), as implemented by the facility Project/Experiment Quality Plan (PEQP). Testing inciudes those initial tests mandated by applicable Technical Safety Requirement (TSR) surveiilance requirements (see Chapter 5.0, "Derivation of Technical Safety Requirements") and Operational Readiness Review (ORR) requirements (see DOE O 425.1 and DOE-STD-3006-93). 

DOE O 420.1, Facility Safety, (DOE 1996) Paragraph 4.1.1.2 specifies that safety structures, systems, and components be designed in accordance with a quality assurance program that satisfies 10 CFR 830.120.

Proper implementation of process safety requirements is key to safety assurance at covered process sites. In this chapter we attempted to identify actions prior to and during the implementation of the PSM and RMP process safety programs. Specific actions for each of the 14 PSM elements were highlighted as well as a section on RMP implementation. Finally, a section on lessons learned and information regarding implementation evasion was presented. 

Audits or some equivalent means of operational reviews are an important segment of any quality assurance program. They provide feedback to management and are the means of ensuring a continuous improvement of the system and overall facility. In the context of audits for process safety there is a close linkage with quality assurance. Quality assurance encompasses the spectrum of not only ensuring that the requirements are met, and it also provides a system to achieve continuous improvement, in addition to compliance.
