Operational Readiness Review

Once the operational readiness review described in the previous chapter is complete the facihty can be started up. Start-up phase is often a hazardous time because the facility is in a dynamic state with conditions varying all flie time, and because management and the operators may not have... 

The purpose of a prestartup review (PSR) is to ensure that initial start-ups, or start-ups following major project work, proceed safely and smoothly. Alternative names for the same activity are prestartup safety review (PSSR) and operational readiness review. As with most process safety activities, a PSR will generally be performed by a small team made up of representatives from operations, maintenance, engineering, and safety. 

Some of these subcommittees, for example, those to do with process hazards analysis and with asset integrity, will be very active for most of the time and will need substantial resources. Others, such as the Operational Readiness Review committee, may not have much to do except at... 

Some elements of the standard, such as operating procedures and mechanical integrity, lend themselves well to pilot projects. Other elements, such as employee participation and operational readiness reviews, are less amenable to this approach. 

B. Level B items must be closed out before Commissioning is complete. However, all of these items must be corrected before the Operational Readiness Review (ORR) commences. 

Safety Management and Regulation Group. It will cover topics such as worker training, job planning, quality assurance, audit, inspection, operation readiness review, and regulations. 

Initial startup testing procedures have been prepared and implemented to demonstrate that structures, systems, and components (SSCs) and processes will perform as intended. Initial testing includes, as appropriate, bench tests and proof tests prior to installation, mockup tests, pre-operational tests, post-maintenance tests, post-modification tests, and operational startup tests. Safety-related items are subject to the quality-assurance requirements of SNL/NM Research Reactor and Experimental Programs (RREP) Quality Assurance Program Plan (SNL 1998a), as implemented by the facility Project/Experiment Quality Plan (PEQP). Testing inciudes those initial tests mandated by applicable Technical Safety Requirement (TSR) surveiilance requirements (see Chapter 5.0, "Derivation of Technical Safety Requirements") and Operational Readiness Review (ORR) requirements (see DOE O 425.1 and DOE-STD-3006-93).

The purpose of a Prestartup or Operational Readiness Review is to ensure that initial startups, or startups following major project work, proceed safely and smoothly. The review team will make sure that management systems are in place, and have been followed, such that the plant will operate safely on restart. 

The PICs of a task or activity must inform the SIMOPs coordinator when their work is done, and that the system has been returned to its normal state (or that an Operational Readiness review is now needed for systems that have been modified). Problems with false assumptions to do with closeout were a major factor in the Piper Alpha accident.

The staff reviewed OPS-SAM-890008, "Reactor Restart Operational Readiness Review Plan" (Reference 1) OPS-SAM-890009, "Reactor Restart Operational Readiness Review Procedure" (Reference 2) and supporting documents to determine the effectiveness of the ORR process to determine the reactor readiness to operate. DOE approved the plan and procedure in June 1990. The staff also reviewed ESH-RSE-90072, "Reactor Operational Readiness Evaluation Plan" (Reference 3), and WSRC-Q4, RSES 203, "K-Reactor Operational Readiness Evaluation Procedure" (Reference 4), which performs an independent evaluation of the ORR process.
