Control Room. The control room location can be critical to the efficient operation of a faciHty. One prime concern is to locate it the maximum distance from the most ha2ardous units. These units are usually the units where LPG or other flammables, eg, hydrocarbons that are heavier than air, can be released and accumulate at grade level. Deadly explosions can occur if a pump seal on a light-ends system fails and the heavier-than-air hydrocarbons coUect and are ignited by a flammable source. Also, the sulfur recovery unit area should be kept at a healthy distance away as an upset can cause deadly fumes to accumulate. [Pg.79]

Condensate Polishing. Ion exchange can be used to purify or poHsh returned condensate, removing corrosion products that could cause harmful deposits in boilers. Typically, the contaminants in the condensate system are particulate iron and copper. Low levels of other contaminants may enter the system through condenser and pump seal leaks or carryover of boiler water into the steam. Condensate poHshers filter out the particulates and remove soluble contaminants by ion exchange. [Pg.261]

In an offshore oil production platform, a major accident occurred partly because pump seals failed and therefore an antifoaming agent was not delivered to a crude oil separator. The fact that the pump seals were defective should have been picked up during routine inspections, but the inspections were neglected because of production pressures. The failure to carry out the inspections was a latent error. [Pg.40]

The change management process should also ensure that a root cause analysis has been conducted to make sure the real problem has been identified and corrected. For example, if a pump seal fails it could simply be replaced with an identical seal. The final element of the equipment description is the equipment boundary figure. A boundary figure is included with each data cell to define the components and limits of the equipment associated with that cell.
