Ketoconazole cream

The ketoconazole 2% cream contains the broad-spectrum synthetic antifungal agent ketoconazole 2%, formulated in an aqueous cream vehicle consisting of propylene glycol, stearyl and cetyl alcohols, sorbitan monostearate, polysor- 

with milling, the remaining ingredients except the fragrance and SD alcohol. Cool with milling (and cooling jacket if needed) to 25°-30°C. 

with mixing, the fragrance and alcohol. Package immediately. 

Charge items 3-5 in a steam-jacketed kettle. Heat to 75°C and then begin stirring to ensure complete melting maintain temperature, keep stirring. 

Charge items 2 and 10 in a separate heating vessel and heat to 75°C add item 6 and stir, preferably under vacuum of 0.5 bar to avoid frothing and add to step 1, passing through a 100-mesh screen by a pump. Rinse with item 10 and add rinsings.

To increase the penetration of ketoconazole into the skin the prescriber sometimes requires the addition of 3 % salicylic acid to ketoconazole 2 % cream. Ketoconazole however is unstable in an acid environment [30]. Addition of salicylic acid to ketoconazole cream leads to an immediate degradation of ketoconazole, causing a blue discolouration. 

In naming pharmacy preparations with more than one active substance, it will in practice not always be possible to foUow the rules about the name and the strength. If for instance 0.1 % triamcinolone acetonide is added to ketoco-nazole cream, the name on the label for the patient would probably be 0.1 % triamcinolone in ketoconazole cream, simply because the full name would not fit.

Vaginal candidiasis is treated by clotrimazole, econazole, isoconazole, ketoconazole, miconazole or nystatin as pessaries or vaginal tablets or cream inserted once or twice a day with cream or ointment on surrounding skin. Failure may be due to a concurrent intestinal infection causing reinfection and nystatin tablets may be given by mouth...

Ketoconazole is alsn used topically in a 2% concentratinn in a cream and in a shampoo for the management of cuta-Ecixis candidiasis and tinea infections. 

Terconazole and butoconazole Terconazole is a ketal triazole with structural similarities to ketoconazole. The mechanism of action of terconazole is similar to that of the imidazoles. The 80-mg vaginal suppository is inserted at bedtime for 3 days, whereas the 0.4% vaginal cream is used for 7 days, and the 0.8% cream for 3 days. Clinical efficacy and patient acceptance of both preparations are at least as good as for clotrimazole in patients with vaginal candidiasis. Butoconazole is an imidazole that is pharmacologically quite comparable to clotrimazole. 

Sample preparation Tablets. Powder tablets, weigh out amount equivalent to about 30 mg, add 100 mL MeOH, sonicate for 5 min, filter. Add a 2 mL aliquot of filtrate to 5 mL of 100 xg/mL ketoconazole in MeOH, make up to 25 mL with MeOH, inject 20 p.L aliquot. Cream. Condition a 500 mg Bond-Elut diol cartridge with 6 mL dichloromethane. Weigh out cream equivalent to about 5 mg of drug, add 30 mL dichloromethane, sonicate for 3 min, make up to 100 mL with dichloromethane, filter. Add a 2 mL aliquot to the cartridge, wash with 2 mL dichloromethane methanol 4 1, wash with 2 mL dichloromethane, elute with 3 mL MeOH buffer 85 15. Add eluate to 0.5 mL 100 p,g/mL ketoconazole in MeOH, make up to 5 mL with MeOH, inject 20 p.L aliquot. (Buffer was 50 mM triethylamine adjusted to pH 7.0 with phosphoric acid.)... 

Other reports " describe essentially similar rises in ciclosporin levels during the use of ketoconazole. The effects of ketoconazole on ciclosporin were found to be slightly increased (from 80 to 85%) when diltiazem was also given. Ketoconazole 2% cream has been found not interact with ciclosporin 1 mg/kg daily in the treatment of contact allergic dermatitis and the ciclosporin dosage does not need to be reduced. Impaired glucose tolerance has been attributed to the use of ketoconazole and ciclosporin in one patient. ... 

A cream containing 2% w/w ketoconazole for cutaneous and mucocutaneous application. Adult dose application to the infected area once or twice a day, continmng for a few days after all visible signs of infection have healed.
