Vaginal candidiasis

Prophylaxis To decrease the incidence of candidiasis in patients undergoing bone marrow transplantation who receive cytotoxic chemotherapy or radiation therapy. Vaginal candidiasis Vaginal candidiasis (vaginal yeast infections caused by Candida). 

Vulvovaginal candidiasis Vaginal (Tablets) 1 tablet (100 mg) at bedtime for 7 days 2 tablets (200 mg) at bedtime for 3 days or 500-mg tablet one time. Vaginal (Cream) 1 applicatorful at bedtime for 7-14 days. 

Miconazole nitrute is supplied in a variety of dosage fonns (cream, lotion, powder, and spray) for the treatment of tinea infections and cuiiineous candidiasis. Vaginal creams and. suppositories are also available for the treatment of vaginal candidiasis. A conceiiiraiion of 2% of the salt is u.sed in moii topical preparations.

Asymptomatic vaginal colonization of Candida albicans is not diagnostic of vulvovaginal candidiasis since 10% to 20% of women are asymptomatic carriers of Candida species. Asymptomatic vaginal colonization does not require treatment.

Daneshmend [104] measured the serum concentration of miconazole in 11 healthy adult females for 72 h following a single 1200 mg vaginal pessary. The mean peak serum miconazole concentration was 10.4 pg/L and the mean elimination half-life was 56.8 h. The mean area under the serum concentration-time curve was 967 pg/L/h. The calculated mean systemic bioavailability of the vaginal pessary was 1.4%. There was large intersubject variation in serum miconazole pharmacokinetics. This formulation may provide effective single dose treatment for vaginal candidiasis.

Tetracyclines inhibit P. acnes, reduce the amount of keratin in sebaceous follicles, and have antiinflammatory properties (inhibiting chemotaxis, phagocytosis, complement activation, and cell-mediated immunity). Drawbacks to tetracyclines include hepatotoxicity and predisposition to infections (e.g., vaginal candidiasis). Other adverse effects include GI disturbances, photosensitivity, tooth discoloration in children, and inhibition of skeletal growth in the developing fetus. Fluconazole is a triazole antifungal that may be administered in recurrent vaginal candidiasis. Fluconazole interacts with sulphonylureas such as glibenclamide, resulting in increased plasma concentrations of the sulphonylurea. 

The use of imidazole antifungal agents such as econazole is the mainstay of treatment in vaginal thrush (candidiasis). 

Use primarily for treatment of patients with progressive and potentially fatal fungal infections. Do not use to treat noninvasive forms of fungal disease such as oral thrush, vaginal candidiasis, and esophageal candidiasis in patients with normal neutrophil counts.
