Knowing what condition the equipment is in so that you can prohibit its use if the condition is unsatisfactory - you will need a Defect Report for this purpose. 

Provide defect reports for reporting details of unserviceable equipment.

The clinical and biological effects and safety of urso-deoxychohc acid in intrahepatic cholestasis of pregnancy have been reported in 19 patients, 14 of whom had clinical improvement, with reduction or disappearance of pruritus, and 11 of whom had an improvement in biochemical hver function tests (13). The only birth defect reported was pyloric stenosis in a boy whose mother had taken ursodeoxychohc acid for 10 days at 34 weeks gestation.

Problem Reporting and Corrective Action. A familiar problem with management systems is that although problems and issues might be effectively and accurately identified and recorded, their resolution can sometimes escape proper control. Procedures need to be in place to ensure that the quality loop is complete so that defect reports are properly tracked to their final resolution.

The site supervisor must ensure that all plant and equipment when delivered to site is in good working order and fitted with all necessary safety devices, notices and guards. It is very often economically justifiable to repair a bad board, if it can be done quickly and with little skilled analysis needed. This is where modem test systems begin to come into their own. More bits of information are needed per bad board a diagnostic test is needed that accurately resolves failures into defect reports that can be acted upon to effect repairs. Because a freshly repaired board will need retesting to ensure it is now defect free, one also would like to find as many defects as feasible in each pass across a tester to avoid a glut of work in progress.
