Bladder ulceration

These drug are used cautiously in patients with renal or hepatic disease, bladder obstruction, seizure disorders, sick sinus syndrome, gastrointestinal bleeding, and asthma Individuals with a history of ulcer disease may have a recurrence of the bleeding. 

O Primary peritonitis develops in up to 25% of patients with alcoholic cirrhosis.3 Patients undergoing continuous ambulatory peritoneal dialysis (CAPD) average one episode of peritonitis every 2 years.4 Secondary peritonitis may be caused by perforation of a peptic ulcer traumatic perforation of the stomach, small or large bowel, uterus, or urinary bladder appendicitis pancreatitis diverticulitis bowel infarction inflammatory bowel disease cholecystitis operative contamination of the peritoneum or diseases of the female genital tract such as septic abortion, postoperative uterine infection, endometritis, or salpingitis. Appendicitis is one of the most common causes of intraabdominal infection. In 1998, 278,000 appendectomies were performed in the United States for suspected appendicitis.5... 

First-generation antihistamines Flypersensitivity to specific or structurally related antihistamines newborns or premature infants nursing mothers monoamine oxidase (MAO) therapy pregnancy (hydroxyzine) angle-closure glaucoma, stenosing peptic ulcer, symptomatic prostatic hypertrophy, bladder neck obstruction, pyloroduodenal obstruction, elderly, debilitated patients (cyproheptadine). 

Use with caution in conditions which might be aggravated by anticholinergic therapy (eg, prostatic hypertrophy, stenosing peptic ulcer, pyloroduodenal obstruction, bladder neck obstruction, narrow angle glaucoma, bronchial asthma, cardiac arrhythmias). 

Gl Cholinesterase inhibitors may be expected to increase gastric acid secretion because of increased cholinergic activity. Monitor patients closely for symptoms of active or occult Gl bleeding, especially those at increased risk for developing ulcers. GU Cholinomimetics may cause bladder outflow obstruction.

Bethanechol should not be used in patients with possible mechanical obstruction of the bladder or gastrointestinal tract or when contraction of smooth muscles in these tissues may be harmful (e.g., recent intestinal resection). It is also contraindicated in patients with bronchial asthma, peptic ulcer disease, coronary artery disease, gastrointestinal hypermotility or inflammatory disease, hypotension or marked bradycardia, hyperthyroidism, parkinsonism, or epilepsy. Care should be exercised in administering pilocarpine to elderly patients because it can enter the CNS and affect memory and cognition, even when applied topically to the eye. 

Iran. Flour is used as a food. A decoction of the dried seed is used externally as an emollient and applied on hemorrhoids and infected ulcers. A decoction of the dried seed is taken orally as a diuretic and antipyretic and used for hepatitis, diarrhea, scorbutism, nephritis, bladder inflammation, gout, enema, and its tonic effect. Decoction of the dried seed is applied to the nose to reduce internasal inflammation " ". 

Oman. Decoction of the bark is taken orally for constipation. Barks and leaves are applied externally for skin rash. Fruit resin mixed with the gall bladder of sheep or goat is applied ophthalmically for cataract. The fruit is applied externally for fractured limb. Cataplasm prepared from leaves is applied externally for ulcers . 

Contraindications Bladder neck obstruction due to prostatic hypertrophy, cardiospasm, intestinal atony, myasthenia gravis in those not treated with neostigmine, narrow-angle glaucoma, obstructive disease of the GI tract, paralytic ileus, severe ulcerative colitis, tachycardia secondary to cardiac insufficiency or thyrotoxicosis, toxic megacolon, unstable cardiovascular status in acute hemorrhage... 

Contraindications Active or latent bronchial asthma, acute inflammatory GI tract conditions, anastomosis, bladder wall instability, cardiacorcoronary artery disease, epilepsy, hypertension, hyperthyroidism, hypotension, mechanical GI or urinary tract obstruction or recent GI resection, parkinsonism, peptic ulcer, pronounced bradycardia, vasomotor instability... 

All cholinesterase inhibitors should be used with caution in patients with cardiac conduction problems because vagotonic effects can lead to bradycardia. In addition, caution is warranted in patients with comorbid asthma, chronic obstructive pulmonary disease, bladder outlet obstruction, or seizures, as well as in patients at risk for gastrointestinal ulcers or bleeding (Fuller and Sajatovic 2000). 

Local or topical administration of amphotericin has been used with success. Mycotic corneal ulcers and keratitis can be cured with topical drops as well as by direct subconjunctival injection. Fungal arthritis has been treated with adjunctive local injection directly into the joint. Candiduria responds to bladder irrigation with amphotericin B, and this route has been shown to produce no significant systemic toxicity. 

Atropine Generic Peptic ulcer, irritable bowel syndrome, neurogenic bladder, bronchospasm, preoperative antisecretory agent, cardiac arrhythmias (e.g., sinus bradycardia, postmyocardial infarction, asystole], reversal of neuromuscular blockade, antidote to cholinesterase inhibitor poisoning...

Adverse Effects. When administered directly into the bladder, common side effects include bladder irritation and infection. Systemic administration (immunization) may also cause dermatologic reactions (peeling or scaling of the skin), allergic reactions, inflammation of lymph nodes, and local irritation or ulceration at the injection site.
