If hemorrhagic cystitis occurs, the goals of treatment are to decrease exposure to the offending etiology, establish and maintain urine outflow, avoid obstruction and renal compromise, and maintain blood and plasma volume. Vasopressin is a peptide hormone produced by the hypothalamus and secreted by the posterior pituitary in response to stimulation. Normal stimuli for vasopressin release are hyperosmolarity and hypovolemia, with thresholds for secretion of greater than 280 mOsm/kg and greater than 20% plasma volume depletion. A number of other stimuli, such as pain, nausea, epinephrine, and numerous drugs, induce release of vasopressin. Vasopressin release is inhibited by volume expansion, ethanol, and norepinephrine. The physiological effect of vasopressin is to promote free water clearence by altering the permeability of the renal collecting duct to water. In addition, it has a direct vasoconstrictor effect. Consequently, vasopressin results in water retention and volume restoration. In patients with septic shock, vasopressin is appropriately secreted in response to hypovolemia and to elevated serum osmolarity (R14). [Pg.97]

When gases that are somewhat soluble in a Hquid concentrate are used, both concentrate and dissolved gas are expeUed. The dissolved gas then tends to escape into the atmosphere, dispersing the Hquid into fine particles. The pressure within the container decreases as the product is dispersed because the volume occupied by the gas increases. Some of the gas then comes out of solution, partially restoring the original pressure. Diuretics are needed to return to normal the expanded extracellular volume that other antihypertensive agents produce, such as fluid retention and blood volume expansion, via compensatory mechanisms of the body. The loss of efficacy of antihypertensive agents can be restored if a diuretic is used concomitandy. In the treatment of hypertension, high ceiling or loop diuretics, such as furosemide, ethacrynic acid, and bumetanide, are no more efficacious than the thiazide-type of diuretics. Tray, Truck, and Tunnel Driers In order to accelerate drying, the closet is factory-built with tight walls. It forms a box, and the air is passed by means of a fan over a radiator or over finned tubes and then over the trays. A portion of the air escapes at the discharge opening the remainder is reheated and recirculated. An amount of new air equivalent to the volume discharged is admitted at the fan. Secondary heating tubes are placed in the path of the air to restore their temperature and heat content. In the tray drier, shallow pans 2 ft by 3 ft by 2 in. deep, for example, are placed on a rack, forming part of the drier. In the truck drier, the rack is on wheels, and the whole may be wheeled in and out of the drier. The fermenters are inoculated with 7.5% by volume of a 24-hour old culture of Aspergillus sderotiorum Huber grown at 28°C in 50 ml aliquots of the above described soybean-glucose medium contained in 300 ml Erlenmeyer flasks, placed on a shaker rotating at approximately 230 rpm. The inoculated fermenters are agitated at 1,380 rpm and each aerated with 1 liter of air per minute and at a temperature of 28°C for 47 hours. A silicone antifoam is added when required. Most types of ion-exchange resin suffer some breakdown and volume loss over time because of attrition, excessive heat, or other factors. Water softeners should be inspected annually, and a double backwash procedure should be provided. This generally lifts the broken resin ( fines ) to the top of the bed, where it can be removed and replaced to restore capacity. Allow for 5 to 10% resin operating capacity loss per year because of physical breakdown. Plasma protein fraction 5% is an IV solution containing 5% human plasma proteins. Serum albumin is obtained from donated whole blood and is a protein found in plasma The albumin fraction of human blood acts to maintain plasma colloid osmotic pressure and as a carrier of intermediate metabolites in the transport and exchange of tissue products. It is critical in regulating the volume of circulating blood. When blood is lost from shock, such as in hemorrhage, there is a reduced plasma volume. When blood volume is reduced, albumin quickly restores the volume in most situations. [Pg.634]
