Staff selection processes

The accident investigation will now get the facts as to which people were involved, what equipment was involved, what were the environmental conditions concerning the lighting, noise and work situation, and also what procedures and policies were there that could have prevented the accident. These procedures could be the safety induction training, the staff selection process, the observation of critical tasks being carried out, or the written safe work procedures, among others. 

The importance of the capabilities and qualifications of technical and operations staff at the toller site was emphasized in Chapter 2, The Toller Selection Process. A requisite for the selection process is having sufficient staff with the appropriate technical skills to select the appropriate drugs (Cohen 2003). The Center for Chemical Process Safety (CCPS), a directorate of AIChE, was established in 1985 to develop and disseminate technical information for use in the prevention of major chemical accidents. The CCPS is supported by a diverse group of industrial sponsors in the chemical process industry and related industries who provide the necessary funding and professional guidance for its projects. Define the special tools, processing equipment, jigs, fixtures, and other equipment required to produce the product. (General-purpose tools and equipment need not be specified because your staff should be trained to select the right tool for the job.)... 

The assessment must be designed to be as objective as possible. An assessment that lacks objectivity may yield questionable results. For example, someone who has been involved in the development or use of a management process is less likely to see weaknesses than an independent observer. When selecting your assessment team, make sure to include staff who will not take things for granted and will bring a fresh set of eyes and ears.

Reportedly, the MEG can be fabricated using only conventional tools and techniques already used by many major manufacturers. All the parts and materials to construct an MEG are available commercially. On October 18, 2000, its development was also announced to selected staff personnel of two Senate Committees.
