Silver sulfadiazine

The sulfonamides are often used to control urinary tract infections caused by certain bacteria such as Escherichia coli, Staphylococcus aureus, and Klebsiella-Enterobacter. Mafenide (Sulfamylon) and silver sulfadiazine (Silvadene) are topical sulfonamides used in the treatment of second- and third-degree bums. Additional uses of the sulfonamides are given in the Summary Drug Table The Sulfonamides. 

When administering a sulfonamide for a bum, the nurse inspects Hie burned areas every 1 to 2 hours because some treatment regimens require keeping the affected areas covered witii Hie mafenide or silver sulfadiazine ointment at all times. Any adverse reactions should be reported immediately to Hie primary health care provider. 

P-lactam antimicrobials, gatifloxacin, indinavir, isoniazid, levofloxacin, nitrofurantoin, ribavirin, rifabutin, rifampin, silver sulfadiazine, streptomycin, sulfonamide antimicrobials, and tetracyclines... 

See Table 47-4 for systemic treatment of an infected pressure sore. A short, 2-week trial of topical antibiotic (silver sulfadiazine or triple antibiotic) is recommended for a clean ulcer that is not healing or is producing a moderate amount of exudate despite appropriate care.

Silver and its compounds have long been used as antimicrobial agents in medicine. The mechanisms of silver toxicity as they relate to human exposure to pharmaceuticals have been reviewed (328). Silver is active at low concentrations and has a low toxicity. The practice of instilling the eyes of infants with 1% of AgN03 solution immediately after birth is still common in some countries, for prevention of opthalmia neonatorum (329). Silver sulfadiazine 77 is clinically used as a topical antimicrobial and antifungal agent and applied as a cream to prevent bacterial infections in cases of severe burns. It is an insoluble polymeric compound and releases Ag(I) ions slowly.

Silver sulfadiazine, a sulfonamide, is used for the management of infected burns. The occurrence of sulfonamide hypersensitivity is a serious risk though and this agent should be reserved for selected cases. Its activity is probably based on the bactericidal action of silver which is released but absorbed only to a negligible extend. The sulfonamide is well absorbed and appreciable blood levels are reached when large areas are treated. 

Contraindications Hypersensitivity to silver sulfadiazine or any component of the formulation... 

Silver sulfadiazine (burn etc. local application SILVIRIN) 1% local cream... 

Sodium sulfacetamide ophthalmic solution or ointment is effective in the treatment of bacterial conjunctivitis and as adjunctive therapy for trachoma. Another sulfonamide, mafenide acetate, is used topically but can be absorbed from burn sites. The drug and its primary metabolite inhibit carbonic anhydrase and can cause metabolic acidosis, a side effect that limits its usefulness. Silver sulfadiazine is a much less toxic topical sulfonamide and is preferred to mafenide for prevention of infection of burn wounds.

Biological and medicinal aspects of silver chemistry are covered in Chapter 62 but a number of features will be outlined here. Silver salts are powerful bacteriocides and it has been known for many years that storing water in silver vessels prevents spoilage. Silver nitrate is highly corrosive and can be applied locally to remove warts or cauterize wounds. In many states in the USA, a 1% AgN03 solution is dropped into the eyes of newborn infants to prevent ophthalmia neonatorum. Silver sulfadiazine has been found effective as a topical application to prevent infections in serious bums victims. 

The mode of antibacterial action appeared to involve interaction with DNA, thus binding the silver.337 Silver sulfadiazine solutions were not light sensitive and unlike silver nitrate did not remove chloride from body fluids.336... 

Sulfacetamide eyedrops continue to be used for treatment of superficial ocular infections. Sometimes silver-sulfadiazine cream is applied to burn surfaces to minimize or prevent bacterial growth, as well as preventing invasive infection.
