An EMEA table contains a series of columns for the equipment reference number, the name of the piece of equipment, a description of the equipment type, configuration, service characteristics, etc, which may impact the fadure modes and/or effects, and aflst of the fadure modes. Whatever the records, they should carry some identification in order that you can determine what they are, what kind of information they record, and what they relate to. A simple way of doing this is to give each record a reference number and a name or title. 

Records can take various forms reports containing narrative, computer data, forms containing data in boxes, graphs, tables, lists, and many others. Where forms are used to collect data, they should carry a form number and name as their identification. When completed they should carry a serial number to give each a separate identity. Records should also be traceable to the product or service they represent and this can be achieved either within the reference number or separately, providing the chance of mistaken identity is eliminated. The standard does not require records to be identifiable to the product involved but unless you do make such provision you will not be able to access the pertinent records or demonstrate conformance to specified requirements.

One method used to summarize the required devices and show the function performed by each device is with a function matrix. Figure 14-4 is a completed function matrix chart for the simple process flow diagram shown in Figure 14-5. The function matrix is from RP I4C and is called a SAFE chart. Each component is listed in the left hand column with an identification number and description. Under Device I.D., each of the devices listed in the SAC is listed. If the device is not present, the appropriate SAC reference number is listed. If the SAC rationale requires that another device be present on another component, that device is listed under Alternate Device, if applicable. 

To locate a citation of a reference in the text, look for the author s name in the author index. The full reference can be found on the page number that appears in bold in the first column the reference number itself appears in italics in the second column and the page(s) of the text where the reference is cited appear in roman type in the. third column. 

SAIC provided much of the data used in this book from its proprietary files of previously analyzed and selected information. Since these data were primarily from the nuclear power industry, a literature search and industry survey described in Chapter 4 were conducted to locate other sources of data specific to the process equipment types in the CCPS Taxonomy. Candidate data resources identified through this effort were reviewed, and the appropriate ones were selected. Applicable failure rate data were extracted from them for the CCPS Generic Failure Rate Data Base. The resources that provided failure information are listed in Table 5.1 with data reference numbers used in the data tables to show where the data originated.

Documentation of data origin is essential. Each completed data collection form needs to contain a file reference number or code to connect it to the documentation sources.
