Malignant fibrous histiocytoma

Malignant fibrous histiocytomas (MFH), often a wastebasket diagnosis in histology, cannot be established by immunohistochemistry. The expression of the histiocyte markers and FXIIIa (originally described as a fibrohisfiocytic marker that is also on dermal dendrocytes) (58) listed in Table 12 in the malignant cells of MFH remains controversial. In addition, these markers are also found in other sarcomas.

ALK-1 may be seen In leiomyosarcoma, rhabdomyosarcoma, and malignant fibrous histiocytoma, but is negative in nodular fasciitis, desmoid tumor, infantile myofibroma-tosis, and infantile fibrosarcoma.

In addition to epithelial neoplasms, a number of sarcomas, CNS tumors, small round cell tumors, and a few germ cell tumors may be positive with EMA. These tumors include malignant nerve sheath tumors, synovial sarcoma, leiomyosarcoma, malignant fibrous histiocytoma, epithelioid sarcoma, and chordoma. With the exception of the last two tumors mentioned, EMA immunostaining is focal.

Carcinosarcoma of the liver can occur, although it is rare. In a series from China in which all of the patients were hepatitis B surface antigen positive, conventional hepatocellular carcinoma merged with areas of rhabdomyosarcoma, malignant fibrous histiocytoma, and fibrosarcoma and immunohistochemistry supported the diagnosis of carcinosarcoma.
