Maintenance Reports

Reports of maintenance on components are potential sources of data on failures, repair limes and unavailability due to maintenance. 

I ole 4.3-2 shows their typical contents. The report may indicate maintenance because of taiiure to operate partially or completely. Such m event is adiicd to the count of component lai lures. 

Preventive maintenance reports provide information on a component s unavailability. They slum the dates of removal from and restoration to service. The frequency can be fotmd from the number of preventive-maintenance reports in a time interval. 

Unfortunately, maintenance reports do not always present all the information indicated in Table 4,3-2. Descriptions of component unavailability or work performed may be unclear, requiring engineering judgment regarding whether a component was made unavailable by maintenance or maintenance was required because of component failure. 

Gaps in a component s data compiled from test and maintenance records can be filled from the conirol room log of events at the plant which records the demands made on components from which operating-time data may be found. Statistical Methods for Nonelectronic Reliability, Reliability Specifications, Special Application Methods for Reliability Prediction Part Failure Characteristics, and Reliability Demonstration Tests. Data is located in section 5.0 on Part Failure Characteristics. This section describes the results of the statistical analyses of failure data from more than 250 distinct nonelectronic parts collected from recent commercial and military projects. This data was collected in-house (from operations and maintenance reports) and from industry wide sources. Tables, alphabetized by part class/ part type, are presented for easy reference to part failure rates assuminng that the part lives are exponentially distributed (as in previous editions of this notebook, the majority of data available included total operating time, and total number of failures only). For parts for which the actual life times for each part under test were included in the database, further tables are presented which describe the results of testing the fit of the exponential and Weibull distributions. 

It is important, especially when consistency has not been designed or built into the maintenance reporting system, to review the data reported to minimize misinterpretation. Clearly defined equipment boundaries for plant hardware are essential for the generation of relevant data. For example, one classification method may define pumps as only the mechanical portions of the pump, whereas another may include the driver (e.g., the motor) and associated controls. Interviews with operating and maintenance personnel as well as review of the maintenance procedures and documents can provide insight into the...

The Engineering department is responsible for the technical documentation, for example, manuals, inspection reports, maintenance reports, specification sheets, etc. These documents are stored and updated by personnel from this department.

Data received through inspection, monitoring and tests should be collected at regular intervals, evaluated and retained. Operation and maintenance reports should be collected and analysed for signs of a deterioration problem. Often this data will be in the form of a technical report. The report should record the data on a daily, weekly, monthly, quarterly... 

Human errors can affect both the cause and the frequency of an event sequence. They can take place before, during or after initiation of the event sequence and can either mitigate or aggravate an accident. These should be modelled in the PSA. Data on human reliability should be derived from sources such as event reports, maintenance reports, PSA reports and simulator observations. 

Maintenance crews should use established company and industry guidelines to determine whether components should be returned to service. They need to determine cause of damage or deterioration. Such analysis may help identify improper use of machinery, or faulty maintenance procedures, which should be corrected. All maintenance or repairs for each vehicle should be documented on a separate maintenance reporting form.

It is important to spend time in pre-planning at inspection. Docrrments on previous inspections, accident investigation and maintenance reports, as well as health and safety committee minutes, should be referred to.

The identification of causes serves no purpose unless there some defensive action is taken. If we apply preventive efforts only to events with the highest likelihood of disaster, we miss the opportunity to prevent a regularly recurring event of low probability which may have latent effects - for example, in the case study, the maintenance reporting and follow through procedures. 

A status color code can be used as an indication of the general status. There may be various selections, for example, three levels of status green, yellow, and red. The green would indicate that SCS/ SBS integrity meets performance requirements, the yellow would demand attention and possible actions to be taken, while a red status would mean that the SCS/SBS is not performing and the risk is increased. The integrity status may be derived from the results of a system function test, preventive/ corrective maintenance reports, etc. 

The required data for this research was collected from operation and maintenance reports and direct field observations over a period of two years. Many of the desired information, such as failure occurrence times, reasons of failure, types of repair actions and time to repair were available in mine database, therefore only TBFs values were calculated. All needed calculations related to data analysis and reliability modeling were carried out using Easy/it and Microsoft Excel software.
