Investigations incidents

Have there been fires, explosions or chemical release incidents If yes, request the incident investigation data, including actions taken to prevent a reoccurrence. [Pg.32]

Incident Investigation Previous incidents related to the chemicals or equipment involved in the new toll should be considered during the PHA and must be considered if subject to PSM/RMP compliance. In addition, procedures should be in place to describe how the client will be informed and involved in the investigation. It is veiy important to ensure that action plans addressing the root cause of past incidents were implemented. [Pg.102]

Ensure the replacement person is given access to the historical records of the toll contract, technology package, MOC packages, and incident investigations. [Pg.122]

The initial incident report is very important. This document captures the initiator s firsthand knowledge of what occurred in the moments after the specific event. Example 5-2, Sample Toller Initial Incident Report, is an example of the data that should be documented as soon as possible. Note that it should be modified using the company s management system procedure and incident investigation procedure, which should describe the type of data needed, and level of detail desired. [Pg.129]

The final incident investigation report written by the assigned incident investigation team (which may include members from the client) can be written in a less structured way depending on the extent of the incident. A detailed discussion of how and why to conduct incident investigations can be found in the AIChE publication. Guidelines for Investigating Process Safety Incidents, Second Edition. [Pg.129]

This form should be used with procedure SWP-041, Incident Investigation Guidelines. Attach additional pages as needed. [Pg.130]

Incident investigation The management process by which underlying causes of undesirable events are uncovered and necessary steps are taken to prevent similar occurrences. [Pg.214]

Incident investigations are a part of process safety management. In investigations, lessons are learned as to how inherently safer technology could have prevented or mitigated the results. How can these learnings be disseminated such that future incidents in similar processes are avoided ... [Pg.128]

Employees in the process area where the incident occurred should be consulted, interviewed, or made members of the team. Their knowledge of the events represents a significant set of facts about the incident that occurred. The report, its findings, and recommendations should be shared with those who can benefit from the information. The cooperation of employees is essential to an effective incident investigation. The focus of the investigation should be to obtain facts, and not to place blame. The team and the investigative process should clearly deal with all involved individuals in a fair, open, and consistent manner. [Pg.242]

The employer investigates incidents that result in, or could result in, a catastrophic release of highly hazardous chemicals. An incident investigation is initiated as soon as possible, but before 48 hours following the incident. An incident investigation team is established to consist of one or more experts in the process involved, and accident investigation. The report prepared at the conclusion of the investigation includes at a minimum ... [Pg.33]

Amendments to the Clean Air Act require employer-employees consultation to develop and implement PSM program elements and hazard assessments. Section 304 requires employee training and education on the findings of PSM incident investigations. [Pg.67]

Where regulations related to process safety exist, they must be understood and PSM system design must be consistent with them. For example, some countries may require that operators participate in hazard analyses and incident investigations. Where there are no process safety-related regulations, you have complete flexibility in system design. [Pg.192]

Another application area is the use of PIFs as part of the process of incident investigation. Any investigation which seeks to establish the imderlying causes of minor or major incidents will benefit from a systematic framework for evaluating the factors which can contribute to the human contribution to such incidents. This topic will also be discussed in Chapter 6. [Pg.104]

For a major incident investigation using a comprehensive root cause analysis system, teams will be formed to acquire information relevant to determine the structure and analyze the causes in depth. In addition to evaluations of the immediate causes, imderlying causes are likely to be evaluated by investigations in areas such as safety and quality management. Both paper- and computer-based systems will be used to acquire and record information for subsequent detailed analyses. [Pg.267]
