Incident Investigation Previous incidents related to the chemicals or equipment involved in the new toll should be considered during the PHA and must be considered if subject to PSM/RMP compliance. In addition, procedures should be in place to describe how the client will be informed and involved in the investigation. It is veiy important to ensure that action plans addressing the root cause of past incidents were implemented. 

A I have asked our Corporate VP for Operations to take day-to-day oversight of the work. I expect to review progress every month. I will make sure that the project has all the resources and management commitment needed to make it succeed. End-of-pipe measures continue to be vitally important. The largest PSM and ESH management costs are accident and incident related. If you reduce the costs of managing PSM and ESH, yet accident and incident rates rise beyond any normal statistical variation, the new system is costing the company more. Near misses are a leading indicator for accidents and incidents and should not be neglected. 

Development of plans for handling accidents and emergencies must precede tlie actual occurrence of these events. In recent years incidents related to tlie chemical, petrochemical, and refinery industries have caused particular concern. Since tlie products of tliese industries are essential in a modem society, every attempt must be made to identify and reduce tlie risk of accidents or emergencies in tliese areas. 

Cost-benefit analysis also requires a determination of the costs incurred from incident-related fatalities. Reduction in individual risk (fatalities per year) is a key component in risk associated with buildings in process plants. One method of determining the cost of fatal accidents is to estimate the amount of money that society might perceive as reasonable compensation in the event of a fatal accident. 

The incident related in sidebar 1.1 points to the purpose of this text—namely, to emphasize the importance of chemical industry production facilities (yes, sugar is a chemical, an organic chemical sugar production qualifies as a chemical production process) being ready for any contingency— any being the key word.

Of the whole amoimt of retired NSs four NSs were withdrawn from service due to incidents related to failmes in heat removal from reactor fuel assemblies while in operation (Echo-II-class NSs 533 and 541 Victor-I-class NS 610 and Alpha-class NS 900) that resulted in drastic worsening of the radiation situation in their RCs.
