Incident investigation second study

Later, while researching for Chapter 24, Measurement of Safety Performance, I observed that companies with superior OSHA rates also had lower workers compensation costs than did other companies. I then wanted to determine whether the quality of investigation of hazards-related incidents would also be superior. (For this second study, it must also be said that the methodology used would not stand the test of good science.)... 

While very few of the incident reports collected in the first study were completed by investigation teams, every report submitted in this second study by three companies was a team effort. In the other three companies, teams are assigned to investigate every OSHA recordable incident. 

Methyl bromide was tested by oral administration in rats and by inhalation in mice and rats. In one 90-day study by oral administration in rats, methyl bromide was reported to produce squamous-cell carcinomas of the forestomach. In a second, 25-week study designed to investigate further the findings of the previous study, early hyperplastic lesions of the forestomach developed after 25 weeks of continuous treatment by gavage. In two inhalation studies in mice, no significant increase in the incidence of tumours was observ ed. In one inhalation study in rats, an increase in the incidence of adenomas of the pituitary gland was observed in high-dose male rats. In another study in rats, no increase in tumour incidence was observed. 

We shall start by looking at Boerhaave s family and his theological education at the Latin School and later at Leiden University. Next we shall investigate the so called incident on the canalboat in which Boerhaave was accused of being a Spinozist. The last part of the chapter deals with a second incident in which Boerhaave was again suspected of Spinozism and scepticism. In this chapter we shall examine how much truth was in the accusations. This means that we also have to look at Boerhaave s position in the contemporary theological and philosophical debates. Finally the incidents clarify why Boerhaave chose to study the creation and how he approached natural philosophy. 

In a study of 32 patients, 84% of those who received paclitaxel developed hypersensitivity reactions characterized by hypotension, respiratory distress, and urticaria (35). These symptoms further confirm that histamine is likely to be the cause of the reaction. The majority of reactions (53%) occurred within 2-3.minutes after the administration of paclitaxel and 78% within 10 minutes. There was one fatal reaction, characterized by hypotension and asystole. Most reactions to paclitaxel occurred after the first or second dose, and hypersensitivity reactions were more common with shorter infusion schedules. Since the duration of the infusion affected the incidence of hypersensitivity reactions, an extension of the infusion duration was investigated. Longer infusion schedules were associated with a reduced incidence of hypersensitivity reactions, the frequency of severe reactions being reduced from 12% or more to 5% with longer infusion times (5,15,49).
