Incident Investigating and Reporting

In an ongoing safety program, incidents and accidents are investigated and analyzed in order to reduce the likelihood of recurrence. The detailed information gained from these investigations is used to develop corrective action. 

The analysis of an accident often includes an estimate of the cost of that accident. This helps in justifying the cost of accident prevention activities. Accidents are generally divided into four classes

Injury/incident investigation and reporting. This document contains the basic procedures and requirements for recording and reporting occupational illnesses and injuries as well as identifying the cause(s) of the accident or near-miss occurrence. In turn, this information can be used to implement corrective changes to preclude the recurrence of similar incidents. 

Workforce Support for Data Collection and Incident Analysis Systems Few of the incident investigation and data collection systems reviewed provide any guidelines with regard to how these systems are to be introduced into an organization.
