Incident investigation first study

The first case study describes the application of the sequentially timed event plotting (STEP) technique to the incident investigation of a hydrocarbon leak accident. Following the analysis of the event sequence using STEP, the critical event causes are then analyzed using the root cause tree. [Pg.292]

In light of earlier evidence that genetic variations in prothrombin of human subjects are clearly associated with the risk of venous thrombosis, and before the results of HERS were known, investigators initiated a case-control study to assess whether prothrombotic mutations modify the association between HRT and the incidence of first MI. Cases were 232 postmenopausal women aged 50 to 79 years who had their first nonfatal MI between 1995 and 1998. [Pg.394]

While very few of the incident reports collected in the first study were completed by investigation teams, every report submitted in this second study by three companies was a team effort. In the other three companies, teams are assigned to investigate every OSHA recordable incident. [Pg.208]

A large number of studies were undertaken to allow optimization of the number, length and location of VRS installed in the roadside and the central reserve of roads fall in the first category. Most of these studies have utilized collated incident data, although the number, quality and relevance of the incidents investigated should be understood, in all cases. The results of these studies will be reported in the next sectiom... [Pg.204]

Three other studies were conducted to investigate the preventive potential of tamoxifen. One in Italy (Veronesi et al. 1998), one at the Royal Marsden Hospital, United Kingdom (Powles et al. 1998), and a multicentric international study (IBIS 2002). The British study was the smallest in size (2471 participants) but concentrated on women with a high incidence of family history and consequently presented a higher number of breast cancers. The Italian trial included only women with previous hysterectomy and, accordingly, around 50% had also undergone bilateral oophorectomy. The family risk was low only 15% had a first-degree relative affected by breast cancer. Both European studies permitted concurrent HRT, and 26% of the participants in the British trial received HRT while on study and 42% had ever received HT for menopausal symptoms. Neither of the studies showed any positive effect of the treatment with tamoxifen on the incidence of breast cancer. Reasons for this lack of effect can be different for each trial. [Pg.259]

The minimum dose of continuously administered nor-ethindrone acetate needed to reduce significantly the incidence of endometrial hyperplasia associated with the use of 17-P-estradiol 1 mg/day has been investigated in a large, controlled, comparative study in 1146 women over 12 months (107). The results suggested that continuous norethindrone acetate at doses as low as 0.1 mg/day is fully effective, at least during the first year of treatment. [Pg.181]

The addition of hydroxychloroquine to sulfonylureas has been investigated in a placebo-controlled study in 125 adipose patients whose diabetes was not well enough controlled with a sulfonylurea alone (179). During the first six months HbAlc was significantly reduced by 1.02%. There were no significant differences in adverse effects, but those who took hydroxychloroquine had a greater incidence of minor corneal changes. [Pg.452]
