The employer investigates incidents that result in, or could result in, a catastrophic release of highly hazardous chemicals. Amendments to the Clean Air Act require employer-employees consultation to develop and implement PSM program elements and hazard assessments. Section 304 requires employee training and education on the findings of PSM incident investigations. 

Where regulations related to process safety exist, they must be understood and PSM system design must be consistent with them. For example, some countries may require that operators participate in hazard analyses and incident investigations. Where there are no process safety-related regulations, you have complete flexibility in system design.

Do all of the required action items uncovered in reactive chemistry testing, hazard analyses and lessons learned from previous incident investigations. 

Check that all items from hazard analyses, incident investigations and other discovery processes have been properly implemented and documented. 

These goals are listed in the order of importance to a company. Of greatest importance is to get near misses reported so that investigation can be used to learn from the incident. Note that incident investigation techniques are essentially the same whether applied to chemical reactivity hazards or to other hazards. 

General guidelines for conducting process incident investigations are given by CCPS (2003). Specific practices that relate to the nature of chemical reactivity hazards are discussed below.

OSHA PSM-covered facilities are required to investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of a highly hazardous chemical in the workplace (29 CFR 1910.119 [m] [1]). At the conclusion of an incident investigation, the company is required to prepare a report on the factors that contributed to the incident. At present, OSHA does not require submittal of these incident reports. However, mandatory submission of the reports would increase available data and thus improve the capability of identifying or tracking reactive incidents.

On the other hand, there are a number of elements of fire prevention that overlap with elements of other programs. For example. Incident Investigation, Management of Change (MOC), Process Safety Information (PSI), and Process Hazard Analysis (PHA) input required for fire prevention can be derived from these same elements of the Process Safety Management (PSM) program. 

Other examples of inductive tools that have limited application in incident investigation include failure mode and effects analysis (FMEA), hazard and operability study (HAZOP), and event tree analysis (ETA). These are detailed in the CCPS book, Guidelines for Hazard Evaluation Procedures... 

Reliance on outside expertise may be the most feasible option for some of these issues. The incident investigation team may act as facilitators and advisors in a similar mode to a PHA (process hazard analysis) study. The outside expert would supply the failure mode information on which possible failures are credible. 

The MORT technique has received domestic and international recognition, and has been applied to a wide range of projects from investigation of occupational incidents to hazards identification. It is supported by detailed documentation, and has been subjected to continued development efforts since it was originally introduced. Today there are several predefined trees available from public and proprietary sources that are based, at least in part, upon tbe MORT tool.
